Book excerpt: Based on the evaluation of the conditions as they existed on the dates of inspection (May 17 and 30, 1978), as revealed by the visual observations, and the review of available information, the condition of Indian Lake Dam is assessed to require immediate further investigation to determine its structural stability and appropriate measures for controlling seepage through the embankment. Field observations show that wet areas and slope irregularities exist on the downstream face of the dam, which suggests potential instability of the embankment. Various seepages on the left abutment also require further evaluation. The outlet pipe gate, which is the only emergency drawdown facility for the impoundment, is inoperative and should be repaired immmediately. Based on the recommended spillway evaluation procedure, the capacity of the spillway was found to be adequate to pass the recommended flow of probable maximum flood. It is recommended that the owner should develop a formal warning system to alert the downstream residents in the event of emergencies.

Book excerpt: In the late 1980s, the National Cancer Institute initiated an investigation of cancer risks in populations near 52 commercial nuclear power plants and 10 Department of Energy nuclear facilities (including research and nuclear weapons production facilities and one reprocessing plant) in the United States. The results of the NCI investigation were used a primary resource for communicating with the public about the cancer risks near the nuclear facilities. However, this study is now over 20 years old. The U.S. Nuclear Regulatory Commission requested that the National Academy of Sciences provide an updated assessment of cancer risks in populations near USNRC-licensed nuclear facilities that utilize or process uranium for the production of electricity. Analysis of Cancer Risks in Populations near Nuclear Facilities: Phase 1 focuses on identifying scientifically sound approaches for carrying out an assessment of cancer risks associated with living near a nuclear facility, judgments about the strengths and weaknesses of various statistical power, ability to assess potential confounding factors, possible biases, and required effort. The results from this Phase 1 study will be used to inform the design of cancer risk assessment, which will be carried out in Phase 2. This report is beneficial for the general public, communities near nuclear facilities, stakeholders, healthcare providers, policy makers, state and local officials, community leaders, and the media.

Book excerpt: Based on a visual inspection, operational history, and available engineering data, the dam is considered to be in fair condition. Structural deficiencies noted during the inspection include an inoperable drawdown mechanism, seepage and ponded conditions at the downstream toe, dense overgrowth (bushes, weeds, small trees) on the down stream slope, and minor concrete deterioration in the spillway. The size classification of the facility is intermediate and its hazard classification is considered to be significant. In accordance with recommended guidelines, the Spillway Design Flood (SDF) for this facility is 1/2 the Probable Maximum Flood (PMF). Results of the hydrologic and hydraulic analysis indicate that the facility can accommodate approximately 32 percent of the PMF and/or 64 percent of SDF (1/2 PMF). Thus, the spillway system is considered inadequate, but not seriously inadequate.

Book excerpt: Based on the visual inspection, operational history, and available engineering data, the dam, despite being well maintained, is considered to be in poor condition. Observations made during the visual inspection suggest that the embankment may be structurally unsafe as extensive areas of seepage were noted on the downstream embankment slope and immediately below the toe area to the right of the outlet conduit. Seepage conditions became evident shortly after reservoir filling and have persisted in spite of the installation of a remedial sand drain system near the toe. Failure of the embankment does not appear imminent and the condition is presently considered non-emergency. It is recommended that the owner immediately develop and implement a warning system to notify downstream inhabitants should hazardous conditions develop. Included in the plan should be provisions for around-the-clock surveillance of the facility during periods of unusually heavy precipitation.

The embankment is reportedly comprised of 3 sections which include an impervious core, a semi-pervious upstream section and a pervious downstream section. The facility is served by a chute channel spillway, with discharges controlled by a free overfall, concrete weir structure. The spillway is located near the right abutment, and has a crest length of 39 feet. In addition, the facility is equipped with a 18-inch diameter steel pipe encased in concrete used as a low level outlet as well as a 16-inch diameter steel siphon which was installed in 1973.

Book excerpt: Based on the evaluation of the existing conditions on March 14 and April 16, 1979, the condition of Little Blue Run Dam is considered to be good. No conditions were noted at this time that would significantly affect the structural performance of the dam. The flood discharge facilities for the dam were designed to pass the probable maximum flood (PMF) without overtopping the embankment. Therefore, according to the recommended criteria, the flood discharge facilities of the dam are adequate. Presently, the pool elevation is approximately 170 feet below the dam crest level. The pool elevation is expected to reach spillway crest level in about the year 2010. The following recommendations should be implemented on a continuing basis: (1) The cracking observed on the crest should continue to be evaluated by the design engineers. Also, attention should be given to the slump and seepage observed on the left abutment. Erosion ditches on the right abutment should be filled; and (2) A formal warning system should be developed to alert the downstream residents in the event of emergencies.

The overall appearance and general condition of the dam and appurtenances are fair. The spillway crest has been raised and the dam crest is two feet below design elevation. These and other noteworthy deficiencies noted. The reservoir pool was 3.5 feet below spillway crest (corewall) on the day of the initial inspection, 22 October 1980. The lake was purposely drawn down so that maintenance of the dam could be performed. On the date of the review inspection, the reservoir level was one foot above the spillway crest (corewall).

Book excerpt: Lake Stoneycreek Dam consists of an earth embankment 900 feet long, with a maximum height of 31 feet from the downstream toe. The dam has a concrete lined primary spillway over the embankment near the left abutment (looking downstream) and an earth channel emergency spillway around the right abutment. Based on evaluation of the conditions as they existed on the dates of inspection (May 18 and 30, 1978), as revealed by the visual observations, and the review of available information, the condition of Lake Stoneycreek is considered to be fair. Although the primary and emergency spillways of the dam have sufficient cross-sectional area to pass the recommended spillway design discharge (probable maximum flood) without overtopping, the emergency spillway crest lacks erosion protection and has no well-defined discharge channel. Therefore, flow over this spillway will constitute a threat to the integrity of the earth embankment. It is recommended that the owner should take appropriate action as soon as possible to provide adequate erosion protection in the emergency spillway, evaluate the adequacy of the riprap at the entrance of the primary spillway, and remove brush on the upstream face of the dam. It is also recommended that the owner should evaluate the need for placing an upstream control at the outlet pipe and installing instrumentation in the embankment.

Tamarack Lake Dam 'A' is located at the northern end of the reservoir and consists of a main embankment, emergency spillway channel, riser intake and outlet conduit, and a diversion dam located to the left of the emergency spillway channel. Tamarack Lake Dam 'B' is located at the southern end of the reservoir and consists of a main embankment, emergency spillway channel, riser intake and outlet conduit, and a saddle dike located to the left of the emergency spillway channel. Tamarack Lake Dam 'A' is 22 feet high and approximately 1000 feet long. A clay core and cutoff trench extend throughout the length of the dam. The minimum width of the core and cutoff trench is 10 feet. The cutoff trench varies in depth below the original ground surface from 4 to 13 feet with the lowest elevation (1195.0 feet) near the location of the outlet conduit.
