Book excerpt: L'allaitement maternel exclusif pendant 6 mois est devenu un enjeu de santé publique majeur ces dernières années pour l'OMS. 31% des mères ont fait le choix de l'allaitement artificiel à la sortie de la maternité en 2015. L'objectif de notre étude était de comprendre les motivations de ces mères dans leur choix de l'allaitement artificiel, et d'étudier le vécu de ce choix dans notre société pro-allaitement. Etude qualitative parentretiens semi-dirigés. La population cible était des mères ayant fait le libre choix de l'allaitement artificiel dès le premier enfant. Un codage longitudinal puis transversal a été effectué. Toutes les analyses des entretiens ont été croisées permettant une triangulation des données. Dix-neuf entretiens ont été réalisés. Les mères avaient de nombreuses représentations négatives de l'allaitement maternel. Elles le considéraient chronophage, les privant ainsi de loisirs et de vie sociale, ou encore comme archaïque ou animal. Les seins étaient considérés comme un objet de séduction et de sexualité en inadéquation avec un possible rôle nourricier. La place du père apparaissait également comme un facteur majeur dans cette décision, revendiquant une égalité père/mère, désirant l'impliquer dans la vie de l'enfant et souhaitant un partage des tâches. Ce choix apparaissait comme inné et instinctif, les mères s'étant peu informées par elles-mêmes ou auprès des professionnels de santé. Certaines mères se sont senties jugées par les professionnels de la périnatalité et la société, exerçant sur elles une pression culpabilisante, alors que dans d'autres cas, ce choix leur paraissait complètement accepté s'inscrivant dans une évolution de la société. Les représentations négatives que les mères avaient de l'allaitement artificiel paraissent finalement plus déterminantes dans leur choix que les avantages de l'allaitement artificiel décrits. Le rôle des professionnels de la périnatalité apparait également limité dans ce choix. Ainsi, les professionnels de santé doivent adapter leur pratique en s'informant tout d'abord auprès des mères sur leurs représentations des allaitements, maternel ou artificiel, sur leurs inquiétudes et leurs préoccupations. Il apparait nécessaire qu'ils instaurent un dialogue autour de ces représentations, sans jugement sur le ressenti des mères. Ainsi, une discussion sur ces sujets pourrait convaincre certaines d'allaiter. Cependant, une fois le choix de l'allaitement artificiel fait, les mères non allaitantes, se sentant parfois isolées et exclues, doivent pouvoir bénéficier d'un soutien, d'un accompagnement et d'informations de la part des professionnels de la périnatalité

In this chapter the author describes three points of epistemological congruence: firstly, dualism is no longer a satisfactory solution; secondly, cautions for the centrality of interpretation (hermeneutics); and, thirdly, the self-criticism of neuroscientists. David W.Mann in his contribution “The mirror crack’d: dissociation and reflexivity in self and group phenomena” tries to show how reflexive processes generate each of three levels of the human system (self, relationships, group) and integrate them one to another, while dissociative processes tend throughout to pull them apart. Health and illness within the self, the relationship and the group can be understood as special states of the dynamic equilibria between these cohesive and dispersive trends. In “Sleep, memory and plasticity” Matthew P. Walker and Robert Stickgold outline a review of the researches following the discovery of rapid eye movement (REM) and non-REM (NREM) sleep, and specifically of those that began testing the hypothesis that sleep, or even specific stages of sleep, actively participated in the process of memory development. The last two chapters, “Clinical implications of neuroscience research in PTSD” by Bessel A. Van Der Kolk, and “Dysregulation of the right brain: a fundamental mechanism of traumatic attachment and the psychopathogenesis of PTSD” by Allan N. Schore, demonstrate how the psychopathology of traumatic conditions can be a fertile field of dialogue between neuroscience and psychoanalysis.

Fred Busch ponders if it has been fully recognized that some post-Bionions suggest a new, radical paradigm for what is curative in psychoanalysis. After searching for the roots of the analyst’s use of reverie in Bion’s work and questioning whether in this regard Bion was a Bionian, Busch carefully examines the work of some post-Bionians and finds both convincing ways to think about the usefulness and limitations of the analyst’s use of reverie. He explores questions including: From what part of the mind does a reverie emerge? How does its provenance inform its transformative possibilities? Do we over-generalize in conceptualizing what is unrepresented, with the corresponding problem of false positives? Do dreams equal understanding and what about the generalizability of the co-created reverie? Busch concludes that it is primarily through the analyst’s own associations that the reverie’s potential is revealed, which further helps the analyst distinguish it from many other possibilities, including the analyst’s countertransference. He believes in the importance of converting reveries into verbal interpretations, a controversial point amongst post-Bionians. Busch ends with the difficult task of classifying the analyst’s reveries based on their degree of representation. The Analyst's Reveries will be of great interest to psychoanalysts and psychoanalytic psychotherapists.
