Notre objectif a été d'étudier la prévalence, les caractéristiques et le pronostic à 5 ans de l'insuffisance cardiaque à fraction d'éjection conservée auprès d'une large cohorte de patients hospitalisés pour leur première poussée d'insuffisance cardiaque. Méthodes : L’ensemble des patients (n=799) hospitalisés pour leur première poussée d’insuffisance cardiaque, durant l’année 2000, dans la Somme, ont été inclus prospectivement. La fraction d'éjection a été évaluée chez 662 patients (83%), qui représentent la population de notre étude. Malgré des recommandations récentes et des études de grande ampleur, elle est mal décrite et pose toujours de nombreux problèmes diagnostiques et de prise en charge thérapeutique. Elle est caractérisée par une grande diversité d'anomalies cardiaques et extra-cardiaques. Surtout, certaines formes pourraient être liées à des causes spécifiques, alors que d'autres formes semblent s'inscrire dans un contexte systémique plus général.Méthodes :Nous avons réalisé une extraction des données de l'entrepôt de santé de l'Hôpital Européen Georges Pompidou grâce à la codification PMSI afin de constituer une cohorte de patients en insuffisance cardiaque à fraction d'éjection préservée (>50%). Cette population a été décrite en catégorisant les étiologies sous-jacentes selon les dernières recommandations de 2019 de l'HFA. La population a été classée en insuffisance cardiaque à fraction d'éjection préservée d'origine primaire et secondaire. Les causes secondaires ont elles-mêmes été séparées en maladie du myocarde et anomalies du remplissage. Nous avons décrit les caractéristiques de la population et réalisé des analyses univariées entre les différents sous- groupes.Résultats :Entre janvier 2016 et décembre 2019, 783 patients sont rentrés dans les critères d'insuffisance cardiaque à fraction d'éjection préservée. Les femmes âgées comorbides étaient majoritaires (56% de la cohorte, moyenne d'âge 80,0 ans, 73% d'HTA, 56% de tachycardie supra-ventriculaire). La cause secondaire représentait 44% de cette population et était caractérisée par des patients plus jeunes que la cause primaire (76.5 ans vs 82.8 ans (p0.001)) avec moins de facteurs de risque cardio-vasculaire (HTA = 62% vs 81% (p
