Book excerpt: La tesis doctoral "Meta-análisis y evaluación científica de programas de drogodependencia" nos ofrece una pormenorizada revisión de varias de las principales cuestiones relacionadas con el complejo problema de la adicción a las drogas. Comienza con una revisión de las principales teorías que han tratado de explicar desde diferentes perspectivas (fundamentalmente la sociológica y la psicológica) el problema de la adicción a sustancias. A continuación se centra el análisis en la situación específica de Puerto Rico, tanto desde el punto de vista de la perspectiva histórica del incremento del consumo como desde la relación que se establece entre éste y la delincuencia. A continuación se revisan diferentes programas y modelos de tratamiento que se han implementado en Puerto Rico para el cuidado y tratamiento de los drogodependientes en diferentes momentos, ofreciéndose una valoración crítica de cada uno de ellos señalando cuáles son sus puntos fuertes y sus puntos débiles y la legislación en que se apoyan. De este análisis se sigue que su eficacia no resulta suficiente y se procede a destacar el incremento de la prevalencia del consumo de drogas en Puerto Rico. Se señala como característica importante su comorbilidad con diferentes trastornos mentales, del comportamiento y de la personalidad. A partir de ahí se plantea la conveniencia de tomar en consideración otro tipo de alternativas intermedias entre la legalización y la prohibición del uso de drogas. En las conclusiones se plantea un tratamiento integrado junto con la inclusión de los servicios de salud tenderá a reducir la demanda de drogas ilegales y, como consecuencia, la disminución de la oferta de este tipo de sustancias.

In the search for the compound which would meet this very natural desire, many drugs have been manufactured and the temptation for the physician is either to increase the dose of a drug which seems to be, at any rate, partially effective, or to add one or more drugs to those which the patient is already taking. Although there have been some references to the dangers of overdosage of drugs for migraine in the past, it was not until relatively recently that it was recognized that drugs given for the relief of headache, if taken injudiciously, may themselves cause headache. The first drugs to be implicated in this way were ergotamine and phenazone. In the case of ergotamine tartrate, the dangers of ergotism were well known as this was a disorder which had been known and written about for many years. In the treatment of headache, fully blown ergotism is rare and in recent years has usually been due to self-medication in doses much greater than those prescribed although there are a few recorded cases where toxic amounts have been given.
