As for AIME 87, the goal of AIME 89 was to promote scientific interchange within and between all subfields of AI in medicine, among researchers from all over the world, and especially from Europe. There were sessions on: knowledge elicidation and acquisition, architectures for medical knowledge-based systems, clinical applications, methodology, reasoning based on physiological models, and uncertainty. It is clear form the quality of papers presented, that the rate of development which took place between the Pavia meeting of 1985 and AIME 87 has been well maintained. With the launch of the European Community's exploratory programme in Advanced Informatics in Medicine in Europe, 1989 is clearly a very important year for this discipline. AIME 89 provided an important forum which demonstrated progress in some of the more difficult methodological problems, and advances in the application of these techniques to real-world medicine. This volume should be consulted by anyone who wishes to appreciate the state of the art in Medical AI in Europe.

Only the technical aspects of computer hardware, software, and communications are discussed as they are necessary to explain how the technology was applied. This approach generally led to defining the objectives for applications of medical informatics. At the end of each chapter, the author summarizes his personal views and interpretations of the chapter contents. Although the concurrent evolution of medical informatics in Canada, Europe, and Japan certainly influenced workers in the United States, the scope of this historical review is limited to the development of medical informatics within the United States. Furthermore, this review is limited to electronic digital computers; it excludes mechanical, analog, and hybrid computers.

The MIE 91 Congress was organized by the European Federation for Medical Informatics (EFMI) in cooperation with the Austrian Computer Society (OCG) and the Austrian Society for Biomedi cal Engineering (OGBMT). It follows the previous congresses in Cambridge (1978), Berlin (1979), Toulouse (1981), Dublin (1982), Brussels (1984), Helsinki (1985), Rome (1987), Oslo (1988), and the Congress 1990 in Glasgow. The proceedings contain 199 contributions to the MIE 91 Congress. They cover all presentations which are part of the scientific programme of MIE 91, among them 157 paper presentations with an average of five pages, 28 poster presentations again with an average of five pages, and 14 abstracts of demonstrations with an average of one page. The papers included were selected by an International Programme Committee out of over 300 submissions after careful review by at least two international reviewers (for whose estimable efforts we are especially thankful). The recommendations of the re viewers were incorporated in the final texts. Some papers were reworked by a professional translator to obtain a high quality of presentation. Several submissions could not be considered for presenta tion at MIE 91 because of shortage of congress time and limitations in the number of pages of the proceedings.

The program committee of the institute consisted of H.K. Huang (Director), Osman Ratib, Albert Bakker, and Gerd Witte. This institute brought together approximately 90 participants from 15 countries. These proceedings are the accumulation of eight years of research and development results in PACS by various dedicated groups throughout the world. The purpose of this institute was to review the most recent technology available for PACS and some clinical results. The readers should notice the remarkable advances in this field by comparing the contents in these proceedings with those in a previous institute on "Pictorial Information Systems in Medicine" held August 27 - September 7, 1984 in Braunlage/Harz, Federal Republic of Germany, and published as Vol. 19 in this series. The institute was organized according to four categories: PACS components and system integration, PACS and related research in various countries and manufacturing companies, clinical experience and research support, and participants' scientific communications. In PACS components, we included image acquisition, workstations, data storage and networking. In system integration, topics on interfaces between Hospital Information System (HIS), Radiology Information System (RIS) and PACS, clinical reports, the ACR/NEMA standard, databases, reliability, and system integration were discussed. This lecture series emphasized the technical detail and "how to" aspects.

Smalltalk80 is an object-oriented programming system which permits rapid prototyping. The need for prototyping in the specification of general practitioner systems was highlighted as long ago as 1980 [4] and is essential to the user -centred philosophy of the project. The goal is a hardware independent system usable on any equipment capable of supporting an integrated environment for handling both textual and graphics and 'point and select' interaction. The architecture is extensible and provides a platform for future experimention with technical advances such as touch screens and voice technology. User Interface Management Systems (UIMS) technology is developing rapidly offering a number of techniques which allow the abstract design of the interface to be separated from the screen/display management on one hand and the internal workings of the application on the other. [2] The importance of this 'layered' approach is that such techniques enable the user to tailor the application to his/her individual preferences and the design team has included and developed many of these ideas into the design. 7. This workshop was sponsored by the EC under the framework of the Medical and Health Research Programme. The aim of the workshop was to assess whether there was sufficient support in the Medical Informatics community in the EC to establish a concerted action. This proceedings contain papers of the presentations given at the workshop. These presentations were centred around three themes: • Methods and Tools • Applications in the domains of chronic care and critical care • Evaluation of decision support systems The papers were prepared after the workshop and therefore we were able to include the relevant parts of the discussions which were related to the presentations. As a result of the discussions during the workshop, a proposal was prepared for the establishment of a concerted action, specifically addressing the development of guidelines for the evaluation of medical decision aids. This proposal was granted early 1990 under the same Medical and Health Research programme of the EC. Over 40 institutes are participating in this concerted action. It have been the outstanding presentations and the open discussions at the workshop that have been the starting point of this concerted action. The papers in this proceedings formed a starting point for the discussions in the meetings of the concerted action.
