Book excerpt: For much of the nineteenth century and all of the twentieth, the per capita rate of suicide in Cuba was the highest in Latin America and among the highest in the world--a condition made all the more extraordinary in light of Cuba's historic ties to the Catholic church. In this richly illustrated social and cultural history of suicide in Cuba, Louis A. Perez Jr. explores the way suicide passed from the unthinkable to the unremarkable in Cuban society. In a study that spans the experiences of enslaved Africans and indentured Chinese in the colony, nationalists of the twentieth-century republic, and emigrants from Cuba to Florida following the 1959 revolution, Perez finds that the act of suicide was loaded with meanings that changed over time. Analyzing the social context of suicide, he argues that in addition to confirming despair, suicide sometimes served as a way to consecrate patriotism, affirm personal agency, or protest injustice. The act was often seen by suicidal persons and their contemporaries as an entirely reasonable response to circumstances of affliction, whether economic, political, or social. Bringing an important historical perspective to the study of suicide, Perez offers a valuable new understanding of the strategies with which vast numbers of people made their way through life--if only to choose to end it. To Die in Cuba ultimately tells as much about Cubans' lives, culture, and society as it does about their self-inflicted deaths.

Its medical missions in Haiti and other developing countries have generated good will toward the Castro government, even as humanitarian groups in North America and Europe organize shipments of medicines and medical equipment to Cuban clinics and hospitals plagued by shortages of the most basic supplies. No country’s health care system functions independently of its economy, and over the years, Cuba’s medical services and public health indicators have improved at some intervals and declined at others. Cuban authorities have been closing medical facilities and making other cutbacks in the health budget, amid reported outbreaks of cholera and dengue fever in several parts of the country. The Cuban health care system is facing more upheaval as the country begins to look ahead to a post-Castro Cuba and the changes this could entail.

In this first comprehensive study of tuberculosis in modern Cuba, Kelly Urban analyzes the medical, social, and governmental responses to the highly contagious disease as the island was heading into and emerging from the Revolution of 1959, providing a window onto broad questions of citizens' rights, biomedicine and public health, and political change. Drawing on a diverse range of sources revealing the perspectives of those at the center of power and those on the margins, Urban finds that the Cuban republican state intervened to confront the tuberculosis problem only after coming under intense grassroots pressure. Cuban citizens forged an activist political subculture around tuberculosis, rejecting discourses that blamed the sick for their own illness. This loose coalition of sanatorium patients, tenement dwellers, black public intellectuals, labor organizers, and reform-minded physicians won entitlements to state health care and pressed for other social rights that influenced health. Their critiques of the state's politicized and inefficient tuberculosis program contributed to the declining legitimacy of the Batista government, helping to spur the Revolution and an innovative restructuring of the public health system.
