Book excerpt: The Army Medical Department (AMEDD) Transformation Workshop (ATW) V, which was conducted at the RAND Corporation Washington Office 25-28 May 2004, is described in this report. It includes an analysis and discussion of the workshop results. This workshop continued the assessment, begun in ATWs I-IV, of the medical risks associated with emerging Army operational concepts and the capacity of the AMEDD to mitigate these risks. The principal purpose of ATW V was to continue the process of providing casualty demand estimates that will need to be addressed by the health service support (HSS) system at echelons above the unit of action (UA). AMEDD subject matter experts supported the workshop and examined the ability of an envisioned UA medical structure to support Future Force combat operations that generated 429 casualties in an approximately 100-hour combat simulation provided by the U.S. Army. The HSS system in the UAs was heavily taxed, and the residual demand for evacuation and care at higher echelons was similarly significant. Nearly two-thirds of the casualties were determined to be ready for evacuation to these higher echelons; their disposition will necessarily depend upon capabilities at those echelons. The complete effect of the casualties examined during ATW V on the HSS system is not known, because the HSS system at higher echelons has yet to be fully developed. The results of the workshop will provide information valuable in designing the medical system at those echelons.

It contains an assessment of the AMEDD Transformation Workshop (ATW) V; describes the workshop s organization, objectives, scenario, and analysis methodology; and provides observations and conclusions. The purpose of this workshop was to continue the assessment, begun in ATWs I IV, of the medical risks associated with emerging Army operational concepts and the capacity of the AMEDD to mitigate these risks. ATWs I III focused on medical risk in a Unit of Action (UA) within a theater that had a defined echelons-above-UA Health Service Support (HSS) system (a 44-bed Combat Support Hospital [CSH]). Based in part on these initial workshops, the AMEDD determined that a 44-bed CSH would likely be insufficient, and it set out to determine what HSS system would be required at echelons above UA. ATW IV assessed the medical risk and demand on an echelons-above-UA HSS system that 76 casualties from a single UA would create during a simulated 12-hour battle. The principal focus of ATW V was to continue the process of establishing the casualty demand data that must be addressed by the echelons-above-UA HSS system. It involved four UAs (and supporting units of employment [UE]) with 429 casualties over a 100-hour simulated battle. Thus, the principal purpose of ATW V was to provide analytical support to the AMEDD to assist it in designing the HSS system above the UA level.

Book excerpt: The Army has been transforming itself into what it calls the Future Force, which relies on a combination of rapid operations, dispersed forces, and superior information to enable a medium-weight force to fight with the punch of heavy conventional forces. These new concepts pose considerable challenges for the organizations that support the combat forces on the battlefield, and the challenge is particularly great for medical forces, which must find, stabilize, and evacuate casualties that are spread across a dispersed battlefield. To determine what the new concepts mean for providing medical support to the fighting units, the Army Medical Department (AMEDD) conducted five Transformation Workshops (ATWs) from 2002 to 2004 to identify the challenges the new concepts posed to providing battlefield medical support and to explore what those challenges might imply for medical force structure.

The principal focus of ATW IV was to begin the process of establishing data on the casualty demand that must be addressed by echelons above the Unit of Action (UA) Health Service Support (HSS) system. Thus, the principal purpose of ATW IV was to provide analytical support to the AMEDD to assist it in designing the HSS system above the UA level. One aspect of future Army operations that is of particular importance is the employment of widely dispersed units moving rapidly around the battlefield. These operational concepts potentially pose significant challenges for the units that support the combat elements. The workshop was supported by two teams of subject matter experts (SMEs), who examined the ability of an envisioned UA HSS structure to support Future Force combat operations. The AMEDD Center and School provided casualty data, which were derived from JANUS simulation results. In the scenario that supported the simulation, a Future Combat Systems (FCS)-equipped Maneuver UA was engaged in 12 hours of combat operations against a brigade-equivalent threat force. At the of the workshop, each SME team was asked to answer the following questions: What was the disposition of casualties (casualty outcomes) at the end of the scenario?, What was the status of the HSS system at the end of the scenario?, and How many casualties require further evacuation and treatment at echelons above the UA?

From the editor: "71F, or "71 Foxtrot," is the AOC (area of concentration) code assigned by the U.S. Army to the specialty of Research Psychology. Qualifying as an Army research psychologist requires, first of all, a Ph.D. from a research (not clinical) intensive graduate psychology program. Due to their advanced education, research psychologists receive a direct commission as Army officers in the Medical Service Corps at the rank of captain. In terms of numbers, the 71F AOC is a small one, with only 25 to 30 officers serving in any given year. However, the 71F impact is much bigger than this small cadre suggests. Army research psychologists apply their extensive training and expertise in the science of psychology and social behavior toward understanding, preserving, and enhancing the health, well being, morale, and performance of Soldiers and military families. As is clear throughout the pages of this book, they do this in many ways and in many areas, but always with a scientific approach. This is the 71F advantage: applying the science of psychology to understand the human dimension, and developing programs, policies, and products to benefit the person in military operations. This book grew out of the April 2008 biennial conference of U.S. Army Research Psychologists, held in Bethesda, Maryland. This meeting was to be my last as Consultant to the Surgeon General for Research Psychology, and I thought it would be a good idea to publish proceedings, which had not been done before. As Consultant, I'd often wished for such a document to help explain to people what it is that Army Research Psychologists "do for a living." In addition to our core group of 71Fs, at the Bethesda 2008 meeting we had several brand-new members, and a number of distinguished retirees, the "grey-beards" of the 71F clan. Together with longtime 71F colleagues Ross Pastel and Mark Vaitkus, I also saw an unusual opportunity to capture some of the history of the Army Research Psychology specialty while providing a representative sample of current 71F research and activities. It seemed to us especially important to do this at a time when the operational demands on the Army and the total force were reaching unprecedented levels, with no sign of easing, and with the Army in turn relying more heavily on research psychology to inform its programs for protecting the health, well being, and performance of Soldiers and their families."

The Army Medical Department (AMEDD) has been deeply involved in the overall Army transformation effort since its inception, and it has identified operational medicine issues whose resolution will be critical to its ability to support the Army of the future. The purpose of this research was to develop a method to assess these issues and further investigate a subset of them. Rand Arroyo Center designed and conducted the Army Medical Department Transformation Workshops (ATW) held in April, August, and November 2002. The purpose of the workshops was to initiate an assessment of the medical risks associated with emerging Army operational concepts and the capacity of AMEDD to mitigate these risks. Medical risk is defined generally as the number, severity, and fate of casualties incurred. The workshops examined combat operations of a notional future force, each supported by a different health service support (HSS) structure. Eight hours of simulated combat provided the context for the workshops, generating casualty data to support the analysis of the HSS structure. AMEDD experts determined likely outcomes for individual casualties resulting from the Army simulation of its 2015 future force. It was concluded that, in this particular simulation, the structure and concepts postulated for the evacuation and treatment of future force combat casualties were overwhelmed by 8 hours of relatively low-intensity combat. This report describes the development of issues that provided a basis for the workshops, workshop organization, the composition of the various teams and cells, objectives and issues, the scenario used, and the analysis methodology employed. Finally, the report provides results and observations. (3 tables, 2 figures).
