Book excerpt: In a previous epidemiological study we reported that the prevalence (45.8%) of low-back pain (LBP) and the two-year incidence (25.9%) of LBP in 284 male taxi drivers in Japan was comparable with rates reported for other occupational drivers in which LBP frequently occurs. LBP was significantly related with the level of uncomfortable road vibrations, and, importantly, increased with total mileage. The aim of this study was to measure whole-body vibration (WBV) on the driver's seat pan of 12 taxis operating under actual working conditions. The results were evaluated according to the health guidelines in International Standard ISO 2631-1:1997. Finally, the relation between total mileage and WBV was investigated. The majority of the frequency-weighted r.m.s. accelerations of the taxis fell into the "potential health risks" zone, under ISO 2631-1:1997. It was clear that the taxi drivers were exposed to serious WBV magnitudes. Therefore, occupational health and safety management should be carried out to help prevent adverse health effects in taxi drivers. In particular, reduction of WBV in taxis and shortening of driving time to reduce duration of WBV exposure should be considered. Moreover, because many taxi drivers work 18 h every other day, the shortening of working hours and taking of rest breaks while working should be considered. Frequency-weighted r.m.s. accelerations of taxis had a tendency to decrease as total mileage increased. The relation between total mileage and WBV should be investigated by taking measurements on the floor and the back rest in addition to the seat pan.

With regard to kind, intensity, and duration of the vibration, the "dose" is decisive as to whether the conse quences are detrimental, unmeaningful, or tolerable with respect to health. This law of nature determines the tasks and goals of those whose aim is to safeguard health. Researchers worldwide have been occupying themselves with this many-faceted question for some time: how mechanical vibration affects the human organism and at what point damage occurs. If prevention in occupational medicine is to succeed, it is most important that the gaps in our present knowledge be closed, for if technical preventive measures are to be effective and preventive means in occupational medicine successful, they must be based on reliable and complete findings. Whenever many independent researchers have worked in a given field for a long period, a comprehensive intermediate as sessment is appropriate to evaluate the level achieved and the direction the research is going. In the area of hand-arm vibration, this evaluation has already been carried out, and the response aroused by this particular research report had led to a demand for a comparable evaluation of the research results on whole-body vibration. This report presents clearly and exhaustively the current status of international knowledge, as well as the questions that remain to be answered.

Book excerpt: Prolonged exposure to whole body vibration (WBV) has been associated with prevalence of spinal disorders among operators of vibrating mobile machinery. The study of biodynamic responses of body segments is thus pertinent for our understanding of potential injury mechanisms and designs of interventions. This study concerns seated body biodynamic responses to vertical vibration through measurements at the driving-point and at body segments, and development of an analytical model for prediction of global and localised responses. Experiments were undertaken to simultaneously measure driving-point apparent mass (APMS) and body segment acceleration transmissibility of 12 adult subjects under random vertical vibration in the 0.5-20 Hz frequency range. Measurements were taken at the C7, T5, T12, L3 and L5 vertebral locations along the fore-aft and vertical axes using skin-mounted micro-accelerometers, and at the scalp using a light-weight head strap with a micro-accelerometer. The study involved four sitting postures realised with different combinations of hands position (on the lap or on the steering wheel) and back support (none or a vertical support), and three excitation magnitudes (0.25, 0.5 and 1 m/s2 RMS). Mathematical correction methods were employed to account for skin effects, sensor misalignments, and seat inertia effects.
