Book excerpt: The evaluation of reproductive, maternal, newborn, and child health (RMNCH) by the Disease Control Priorities, Third Edition (DCP3) focuses on maternal conditions, childhood illness, and malnutrition. Specifically, the chapters address acute illness and undernutrition in children, principally under age 5. It also covers maternal mortality, morbidity, stillbirth, and influences to pregnancy and pre-pregnancy. Volume 3 focuses on developments since the publication of DCP2 and will also include the transition to older childhood, in particular, the overlap and commonality with the child development volume. The DCP3 evaluation of these conditions produced three key findings: 1. There is significant difficulty in measuring the burden of key conditions such as unintended pregnancy, unsafe abortion, nonsexually transmitted infections, infertility, and violence against women. 2. Investments in the continuum of care can have significant returns for improved and equitable access, health, poverty, and health systems. 3. There is a large difference in how RMNCH conditions affect different income groups; investments in RMNCH can lessen the disparity in terms of both health and financial risk.

With funding from the William and Flora Hewlett Foundation, the Andrew W. Mellon Foundation, and the U.S. Agency for International Development, the committee organized a workshop on this topic in Washington, DC, on October 19-20, 1998. The Consequences of Maternal Morbidity and Maternal Mortality assesses the scientific knowledge about the consequences of maternal morbidity and mortality and discusses key findings from recent research. Although the existing research on this topic is scarce, the report drew on similar literature on the consequences of adult disease and death, especially the growing literature on the socioeconomic consequences of AIDS, to look at potential consequences from maternal disability and death.

Maternal mortality is thus one of the most serious public health issues facing India today. During the period 1990-2009, Maharashtra’s Maternal Mortality Ratio (MMR) dropped from around 216 to 104, making it a 52% decrease in 19 years. The MMR in Maharashtra is the third lowest in the country. In 2009 Maharashtra became one of the three states (along with Tamil Nadu and Kerala) in India to have achieved the Millennium Development Goal of an MMR of 109 by the year 2015. This remarkable drop in the MMR deserves to be studied in order to identify the reasons for its occurrence and to see whether it can be replicated in the rest of the country.

Abstract: Background: India as one of the rapidly developing economies where health challenges are myriad at the population level has the highest number of maternal death in the world. Understanding risk factors for maternal mortality is paramount because maternal health is the basic indicator for the overall adequacy of healthcare of a country. This study was conducted to review on the various risk factors of maternal mortality and the multifarious challenges for maternal health in India. Methods: A literature search was conducted with PubMed and Google scholar using the key words of ("risk factors" AND ("maternal mortality" OR "maternal death") AND India) for articles published from 1970 to May 2014. PubMed was primarily used for the systematic search. Findings: Twelve studies were identified for the final review of which six were case series studies, three were case studies and three were case control studies. Most of the studies were conducted in institutional settings from the five regions (North, South, West, Central and East) of India with different range of Maternal Mortality Rate (MMR) estimates. Previous literature had highlighted socio economic disadvantages as important determinants for maternal mortality. The current review shows a complex interplay of four factors in general in India: social, obstetrical, behavioural and medical factors. Variables of both social demographic and economic factors such as median age of the women at childbirth, literacy rate of the female population and area of residences are put together in the social factors of this study. Compared to the causes, descriptions on behavioural risk factors were rather limited and so the requisite to examine the risk factors affecting maternal mortality is justified. Intervention strategies include conditional cash transfer scheme, voucher scheme, training of village health volunteers and training of auxiliary mid wives'. Conclusions: India has a unique social system of diversity and stratification. The pattern of maternal mortality in India is different and varied widely in zones or regions. The variations of challenges should be highlighted so as to give a clear grasp of the inequalities of maternal health as well as also help in reducing the MMR substantially. India hosts the greatest number of actual maternal deaths in the world, although Nigeria’s ratio of maternal deaths to live births is greater than India’s. This thesis focuses on the experience of these two countries in order to determine if India’s governmental policies, level of government corruption, and traditional practices directly lead to its comparably better maternal health statistics. India has demonstrated lower ratios of maternal mortality and more consistent improvement in maternal health indicators than Nigeria as a result of stronger political support for maternal health, more effective government policies to improve accessibility of maternal healthcare, and fewer instances of government corruption. Nigeria must increase protections for citizens coming forward with complaints against government practices. Both countries will benefit from doubling their efforts to get the issue of maternal health on the agenda of state-level politicians, as well as strengthening strategies to recruit and train local women to serve in their own communities to improve the urban-rural divide in maternal health indicators.

Worldwide, it is estimated that women are the sole supporters in 18 to 30 percent of all families, and that their financial contribution in the remainder of families is substantial and often crucial. This book provides a solid documentary base that can be used to develop an agenda to guide research and health policy formulation on female health--both for Sub-Saharan Africa and for other regions of the developing world. This book could also help facilitate ongoing, collaboration between African researchers on women's health and their U.S. colleagues. Chapters cover such topics as demographics, nutritional status, obstetric morbidity and mortality, mental health problems, and sexually transmitted diseases, including HIV.
