The resulting book, Noise and Military Service: Implications for Hearing Loss and Tinnitus, presents findings on the presence of hazardous noise in military settings, levels of noise exposure necessary to cause hearing loss or tinnitus, risk factors for noise-induced hearing loss and tinnitus, the timing of the effects of noise exposure on hearing, and the adequacy of military hearing conservation programs and audiometric testing. The book stresses the importance of conducting hearing tests (audiograms) at the beginning and end of military service for all military personnel and recommends several steps aimed at improving the military services' prevention of and surveillance for hearing loss and tinnitus. The book also identifies research needs, emphasizing topics specifically related to military service.

Book excerpt: A standardized process of determining the hearing health readiness of personnel in uniform does not exist across the services in the Military Health System, (MHS). This study examines the interchangeability between the Occupational Safety and Health Administration (OSHA) Reportable Hearing Loss (ORL) and the U.S. Army s H-3 and H-4 profile formula. The Army employs a specific formula for determining the profiling measures for uniform personnel(AR 40-501, 1995). The Air Force also uses a published standard for identifying fitness (United States Air Force Instruction 48-123, 1997). The U.S. Navy and Marine Corps use publications outlining the management of their Hearing Conservation/Industrial Audiology programs, U.S. Navy Instruction 5100.19C Forces Afloat and U.S. Navy Instruction 5100.19D Forces Ashore. These publications do not present criteria for evaluating the readiness of their uniform personnel using a classification system. If interchangeability exists between the ORL and Army s standards, this method may be applied across the Department of Defense to evaluate the hearing health readiness for uniform personnel.

There were no hearing conservation programs in the militaryZ Aural rehabilitation remained the main focus until around 1950, when jet aircraft began to appear at airbases and on aircraft carriers. The surprising increase in level of jet engine noise stimulated studies that verified the threat of high levels of acoustic energy to human auditory and non-auditory function. Intensified efforts expanded this concern to include other military-unique noises and, eventually, conventional occupational noises. In 1947, 23 years prior to the Occupational Safety and Health Agency regulation of 1970, the first hearing conservation program was implemented in the Air Force. Few hearing conservation programs existed in industry. Many present industrial programs are modeled after this pioneering Air Force effort. Today, no total uniformity exists in the hearing conservation programs of the military branches. An effort is nearing completion to integrate these individual programs into a new Department of Defense Hearing Conservation Program. Initial implementation of this revolutionary program is expected during calendar year 1998.
