Book excerpt: "Hypoxaemia is a major contributor to child deaths that occur worldwide each year; for a child with pneumonia hypoxaemia increases the risk of death by up to 5 times. Despite its importance in virtually all types of acute severe illness, hypoxaemia is often not well recognized or well managed, more so in settings where resources are limited. Oxygen therapy remains an inaccessible luxury for a large proportion of severely ill children admitted to hospitals in developing countries. This is particularly true for patients in small district hospitals, where, even if some facility for delivering oxygen is available, supplies are often unreliable and the benefits of treatment may be diminished by poorly maintained, inappropriate equipment or poorly trained staff with inadequate guidelines. Increasing awareness of these problems is likely to have considerable clinical and public health benefits in the care of severely ill children. Health workers should be able to know the clinical signs that suggest the presence of hypoxaemia and have more reliable means of detection of hypoxeamia. This can be achieved through more widespread use of pulse oximetry, which is a non-invasive measure of arterial oxygen saturation. At the same time oxygen therapy must be more widely available; in many remote settings, this can be achieved by use of oxygen concentrators, which can run on regular or alternative sources of power. Having effective systems for the detection and management of hypoxaemia are vital in reducing mortality from pneumonia and other severe acute illnesses. Oxygen therapy is essential to counter hypoxaemia and many times is the difference between life and death. This manual focuses on the availability and clinical use of oxygen therapy in children in health facilities by providing the practical aspects for health workers, biomedical engineers, and administrators. It addresses the need for appropriate detection of hypoxaemia, use of pulse oximetry, clinical use of oxygen and delivery systems and monitoring of patients on oxygen therapy. Initialement, l'utilisation de l'oxygénothérapie haut débit (OHD) était limitée à l'unité de soins intensifs (USI). Récemment, son utilisation s'est étendue aux services d'urgence et de pédiatrie générale, mais il n'y a pas de recommandations claires sur le sujet. Les objectifs de cette étude étaient de décrire la prise en charge des patients nécessitant la mise sous oxygénothérapie haut débit en dehors des soins intensifs et d'identifier les facteurs associés à l'échec de l'OHD en cas de bronchiolite modérée à sévère. Méthodes : Une revue rétrospective des patients âgés de 0 à 6 mois atteints de bronchiolite ayant reçu une OHD au sein de sept hôpitaux pédiatriques tertiaires au cours des 5 dernières saisons épidémiques de 2018 à 2023 a été réalisée. Les variables démographiques, cliniques et biochimiques ont été recueillies à l'admission et au début du soutien thérapeutique par OHD. La prise en charge initiale et son évolution ont été décrites. L'échec du traitement par OHD a été défini comme le transfert en unité de soins intensifs. Une analyse de régression multiple a été utilisée pour déterminer les paramètres associés à l'échec de l'OHD. Résultats : Nous avons inclus 383 nourrissons dans cette étude multicentrique (âge moyen de 63 jours [7 ; 192]). 76 patients (20%) nécessitant un HNFC ont finalement été transférés en USI pédiatrique sous une durée médiane de 2 jours. Seuls 40 enfants (10%) ont bénéficié d'une escalade ventilatoire et parmi les enfants transférés en USI, un seul patient a été intubé. Les principaux facteurs associés au transfert en USI étaient l'âge et le poids à l'admission, la prématurité, l'infection par le VRS, la défaillance neurologique, les scores Wang et m-WCAS élevés, l'hypoxémie, les apnées, l'indice ROX bas, l'acidose, les atélectasies ou la pneumonie, en analyse univariée. En analyse multivariée, l'âge, le terme, l'infection à VRS, les scores de Wang et m-WCAS, la saturation en oxygène à l'admission et l'index ROX restaient associés. Conclusion : L'oxygénothérapie haut débit semble être un outil sûr pour la prise en charge de la bronchiolite modérée à sévère. Cependant, il y a un manque de standardisation de la pratique et d'autres études sont nécessaires pour affiner les directives de l'OHD en dehors de l'USI et pour identifier rapidement les personnes qui ne répondent pas à l'OHD.
