These variations in human behaviour have profound mental and physical implications. The aim of the present work is to provide an overview of the psychology of pessimism. It is composed of four sections - I: How are optimism and pessimism measured?, II: What is the relationship between optimism and pessimism?, III: What is the relationship between optimism-pessimism and health?, and IV: What are the biology and chemistry of optimism-pessimism? For various reasons, rigorous analysis of pessimism has been difficult. Nonetheless, recent advances, including the development of numerous psychological batteries, have allowed for structured, scientific investigation. Indeed, several studies have examined the medical implications and biological and chemical bases of pessimism. Nonetheless, the psychology of pessimism is imperfect and further study is needed.

It helped, for example, in formulating the laws of immunogenetics and in achieving a better understanding of the mechanism governing the fate of allotrans planted tissue. Cancer has often been considered a foreign body, and therefore many attempts have been made to cure it on the basis of the immunological principles applicable to parasitic, bacterial, or viral diseases. Vaccination has been investigated clinically for its po tential to prevent cancer [1], and clinical research into the cure of cancer has included active immunization [2], nonspecific stimulation of defense mechanisms [3], and transfer of passive immunity by cells [4] or by antibodies [5]. These experiments have been of limited value and in some cases have even had an adverse effect. The reputation of cancer immunology has suffered, and the concept has often been severely criticized. However, the basic postulate that cancer must originate from the body's own cells, and that the immune system belongs to the principal regulatory mechanisms, remains valid. In recent years new research into clinical cancer immunotherapy has included critical appraisal of all its potential benefits, and also of the risks and limitations. It is dangerous to make unrealistic promises or to speak or think in terms of "miracle weapons" or a "magic bul let.

Book excerpt: Transplantation of syngeneic (donor is a monozygous twin) or allogeneic (donor is an HLA-identical sibling) marrow provides the opportunity for aggressive antileukemic therapy without regard to marrow toxicity. Until 1975, marrow transplantation was carried out only after failure of all other therapy. Consequently, most patients were in advanced relapse. Six of 16 recipients of syngeneic marrow and 13 of 100 recipients of allogeneic marrow are still in remission after 5. 5-10 years [3, 7]. An actuarial survival curve of the first 100 patients grafted in Seattle after conditioning with cyclophos phamide (60 mg/kg on each of 2 successive days) and total body irradiation (1,000 rad) showed three periods of interest: (1) The first 4 months showed a rapid loss of patients associated with advanced illness, graft-versus-host disease, infections (in particular interstitial pneumonias), and recurrent leukemia; (2) from 4 months to 2 years, the curve showed a much slower rate of decline attributable primarily to recurrent leukemia; and (3) from 2-10 years, the curve was almost flat with a negligible loss of patients and no recurrent leukemia. This flat portion of the curve corresponded to 13% of the patients and indicates a strong probability that the majority of these survivors are cured of their disease [8]. Attempts at reducing the incidence of leukemic relapse after transplantation were made by a number of marrow transplant groups by added chemotherapy.
