Book excerpt: As a result of an aging population, aging baby boomer nurses, and an increased demand for baccalaureate prepared nurses, the demand for future nurses is expected to reach epic proportions. These major changes in the health care system will require profound changes in nursing education as faculty aim to improve teaching approaches in preparing student nurses for practice. In addition, the accrediting body for BSN programs has set minimum required pass rates for first time NCLEX-RN test takers. Inadequate and declining pass rates have the potential to impact nursing programs, stakeholders, and the health care system as a whole. The purpose of this study was to explore nursing faculty sense-making of classroom experiences they believe supportive of NCLEX-RN success. A qualitative approach was used for this study, which followed the research tradition of Interpretative Phenomenological Analysis. This approach allowed the researcher to explore, analyze, and interpret nursing faculty sense-making of lived experiences in the classroom. Data was gathered from participants through use of semi-structured interviews. Super-ordinate themes emerging from the data included; responsibility for student success, affected by application of teaching practices, and motivated by the NCLEX-RN. Findings revealed faculty reliance on student participation in learning as well as a strong faculty desire for student success. Faculty were responsive to diverse teaching practices, energized by integration of knowledge, and experienced uncertainty around NCLEX-RN preparation. In addition, faculty were encouraged by their dedication to NCLEX-RN style questioning and energized to align course content with the NCLEX-RN test plan. Findings from this study add to the current literature around the NCLEX-RN, and may provide nursing faculty, administrators, accrediting bodies, and other stakeholders with insight into valued teaching practices supportive of NCLEX-RN success. Several recommendations for practice and further research are included in the report of findings.

In this book, the Institute of Medicine makes recommendations for an action-oriented blueprint for the future of nursing.

Book excerpt: Today's complex healthcare system requires nursing graduates to be prepared to deliver high quality, patient-centered care to a diverse population. Nurse educators must be highly proficient nurses as well as excellent educators who are able to facilitate student learning. There is a paucity of published literature regarding the optimal preparation for nurses entering the faculty role. This retrospective survey study examined the relationship between the educational preparation of nursing faculty and the student outcomes of program pass rates and performance on key components on the National Council Licensure Examination for Registered Nurses (NCLEX-RN) in 40 associate degree nursing (ADN) programs in Florida, both public and private/proprietary. The specific components of educational preparation that were included in this study were (a) the highest degree attained; (b) the type of doctoral degree attained (Doctor of Education, Doctor of Philosophy, Doctor of Nursing Practice, or other); and (c) completion of a minimum of nine credits of graduate level education coursework covering topics such as curriculum development, instructional design, assessment and evaluation, and adult learning theories. Other faculty characteristics analyzed were teaching experience, student-to-faculty ratio, and percentage of full-time faculty. All faculty data were grouped by program. Faculty characteristics shown to predict program NCLEX-RN pass rates were faculty completion of graduate level education coursework and years of teaching experience (averaged teaching experience by the full-time faculty group within a program), although faculty completion of education coursework was related to NCLEX-RN pass rates at a significantly higher rate than was found with years of teaching experience. Based on the findings from this study, ADN programs in Florida that employ full-time faculty members who have completed graduate-level education coursework may see positive effects on NCLEX-RN pass rates. Likewise, ADN programs in Florida that increase the average teaching experience of the full-time faculty may see positive effects on NCLEX-RN pass rates.

Book excerpt: Graduates of associate degree (AD) nursing programs form the largest segment of first-time National Council Licensure Examination for Registered Nurses (NCLEX-RN) test takers, yet also experience the highest rate of NCLEX-RN failures. NCLEX-RN failure delays entry into the profession, adding an emotional and financial toll to the unsuccessful AD candidate. Studies concerning factors contributing to first-time success on the NCLEX-RN may include AD graduates, but primarily focus on baccalaureate degree nursing program graduates. Commercially available progression specialty examinations and exit examinations attempt to identify students at risk for failure on the NCLEX-RN. In an effort to increase first-time pass rates, many schools of nursing have implemented such examinations to better prepare nursing students for the NCLEX-RN examination. The present study sought to determine whether progression testing, using standardized computerized tests throughout the program, contributed to the success of AD nursing students taking the NCLEX-RN licensure examination for the first time. The study employed a retrospective, descriptive correlational design at a single two-year associate degree nursing program at a private college in the northeast. A convenience sample of the records of 410 nursing students, who graduated from the nursing program in 2008, 2009, 2010, 2011, 2012, and 2013, were identified. The most reliable predictor of NCLEX-RN success was end-of-program grade point average. The study found no significant correlation with the initiation and use of computerized standardized testing and NCLEX-RN outcomes. Lastly, there was no difference in NCLEX-RN outcomes based on student enrollment in the day or evening AD nursing program.

Book excerpt: In 2015, despite pedagogical concerns from Canadian nurse educators, the National Council Licensure Exam Registered Nurse (NCLEX-RN), an American nursing licensure exam, was adopted by Canadian nurse regulators for use in Canada. Initial pass rates for the NCLEX-RN were much lower than traditional pass rates had been on the former Canadian Registered Nurse Exam. As a nurse educator, I found the news of the decline in pass rates very distressing. This dissertation is about how, as a nurse educator, I came to terms with the adoption of the NCLEX-RN. I use an autobiographical narrative inquiry approach to explore the meaning of the adoption of the NCLEX-RN for my educational practice. I reflect on my own experiences of writing the NCLEX-RN and how the adoption of the NCLEX-RN has impacted my pedagogy. Miller’s (2007) theory of a holistic curriculum guides the study. As well, Dewey’s theory about miseducation and the critical perspectives of Paulo Freire and bell hooks inform my study. Via narrative analysis and a holistic lens, I identified how the adoption of the NCLEX-RN impacted my body, mind, and soul. The following seven narrative themes were identified: a) a high price to pay; b) constraining forces; c) awakening to the biases of computerized testing; d) contradictions and consequences; e) finding my pedagogical peace; f) searching for an antidote; and g) reaffirming my teaching self. I raise concerns about the erosion of holistic nursing pedagogies if NCLEX-RN pass rates become the dominant curricular driver. More studies are needed to fully explore the impact of the adoption of the NCLEX-RN on Canadian nurse educators, nursing curricula and nursing care.
