Book excerpt: Informed consent - the process of communication between a patient or research subject and a physician or researcher that results in the explicit agreement to undergo a specific medical intervention - is an ethical concept based on the principle that all patients and research subjects should understand and agree to the potential consequences of the clinical care they receive. Regulations that govern the attainment of informed consent for treatment and research are crucial to ensuring that medical care and research are conducted in an ethical manner and with the utmost respect for individual preferences and dignity. These regulations, however, often require - or are perceived to require - that informed consent documents and related materials contain language that is beyond the comprehension level of most patients and study participants. To explore what actions can be taken to help close the gap between what is required in the informed consent process and communicating it in a health-literate and meaningful manner to individuals, the Institute of Medicine's Roundtable on Health Literacy convened a one-day public workshop featuring presentations and discussions that examine the implications of health literacy for informed consent for both research involving human subjects and treatment of patients. Topics covered in this workshop included an overview of the ethical imperative to gain informed consent from patients and research participants, a review of the current state and best practices for informed consent in research and treatment, the connection between poor informed consent processes and minority underrepresentation in research, new approaches to informed consent that reflect principles of health literacy, and the future of informed consent in the treatment and research settings. Informed Consent and Health Literacy is the summary of the presentations and discussion of the workshop.

In this book, Becky Cox White provides a concise introduction to the key practical, philosophical, and moral issues involved in competence to consent. The goals of informed consent, respect for patient autonomy and provision of beneficent care, cannot be met without a competent patient. Thus determining a patient's competence is the critical first step to informed consent. Determining competence depends on defining it, yet surprisingly, no widely accepted definition of competence exists. White identifies nine capacities that patients must exhibit to be competent. She approaches the problem from the task-oriented nature of decision making and focuses on the problems of defining competence within clinical practice. Her proposed definition is based on understanding competence as occurring in a special rather than a general context; as occurring in degrees rather than at a precise threshold; as independent of consequential appeals; and as incorporating affective as well as cognitive capacities. Combining both an ethical overview and practical guidelines, this book will be of value to health care professionals, bioethicists, and lawyers.

Book excerpt: In Fevered Measures, John Mckiernan-González examines public health campaigns along the Texas-Mexico border between 1848 and 1942 and reveals the changing medical and political frameworks U.S. health authorities used when facing the threat of epidemic disease. The medical borders created by these officials changed with each contagion and sometimes varied from the existing national borders. Federal officers sought to distinguish Mexican citizens from U.S. citizens, a process troubled by the deeply interconnected nature of border communities. Mckiernan-González uncovers forgotten or ignored cases in which Mexicans, Mexican Americans, African Americans, and other groups were subject to—and sometimes agents of—quarantines, inspections, detentions, and forced-treatment regimens. These cases illustrate the ways that medical encounters shaped border identities before and after the Mexican Revolution. Mckiernan-González also maintains that the threat of disease provided a venue to destabilize identity at the border, enacted processes of racialization, and re-legitimized the power of U.S. policymakers. He demonstrates how this complex history continues to shape and frame contemporary perceptions of the Latino body today.

The company's unique concept of clinical research held great promise and had suc cessfully endured many of the fits and starts characteristic of entrepre neurial organizations. With a staff of highly enthusiastic, albeit inex perienced, field personnel located in 30-odd cities around the country, we found ourselves off and running with several critical research pro grams for major pharmaceutical manufacturers. Our excitement with the innovation was tempered with the reality of staffing and bearing responsibility for more than 30 field offices and 300 new staff persons, more than half of whom had no previous experience in the pharma ceutical industry. In the ensuing few years, we explored by trial and error many workable and unworkable patterns of training! delegation, data collec tion, and auditing. The ideas expressed in this book benefited greatly from that experience and from the willingness of our co-workers and clients to share insights and problems. During those years, we also sought guidance from the works available on the clinical trials field. Although we found numerous references on research ethics, little guid ance was available on the practical aspects of conducting a clinical trial.
