Book excerpt: Sinus floor elevation is often applied to the molar region of the atrophic maxilla to increase in bone mass between the alveolar crestand the sinus floor. Although autogenous bone graft is, so far, gold standard, limitations of donor site bone volume and post-operativemorbidity could be disadvantages. Thus, artificially synthesized bone substitutes, hydroxyapatite (HA) and tricalcium-phosphate(TCP), might be considerably useful materials compared to the autogenous bone. We have developed a novel technique of the sinus floor elevation using plate-shaped beta-TCP in order to perform surgery lessinvasively and to reduce the stress of patients. In this study, we evaluated the prognosis of this technique and the newly developedbiomaterials. Thirteen patients were recruited in this study. The diameter and thickness of plate-shaped beta-TCP were approximately 10 mm and2 mm, respectively. The bony windows were prepared on the lateral wall of the sinus and the sinus membrane was carefully removedand elevated. Then the TCP plate was inserted and supported with dental implants or micro-screws. Finally, bio- resorbablemembrane was placed to cover the bony window and to avoid soft tissue infiltration. The panolamic X-ray and the cone beam CT wastaken every year after the operations to evaluate the volume of newly formed bone, the implant stability, and the remodeling of TCPplates. This clinical research was approved by Ethics Committee, School of Dentistry, Iwate Medical University, and the patient'sconsent was also obtained. Thirteen operations of the sinus floor elevation with plate-shaped beta-TCPs have been performed since 2011. This study consistedof three men and ten women. The average age of all the patients was 53.3 years old. Four of those were staged approach and nine ofthose were simultaneous approach. Ten of 13 patients have had no complications concerning the abnormalities of the maxillary sinusfrom the postoperative to the present, and it passed well. In most cases, newly formed bone in the sinus was observed on X-rayphotos 6 month after the operation. The good stability of the dental implant, at the second stage surgery, was confirmed. TCP platesremained in the same position and achieved the secure space making for long time. Panoramic X-ray and X-ray CT analysesrevealed that TCP-plates were resorbed in 5 years post operation in most cases. There were two patients who complain ofuncomfortable feeling and TCP plates were removed surgically. One TCP plate was spontaneously excreted. In this clinical trial, a new bone formation was observed in the empty space between the TCP plate and the original sinus floor. TCP isa bone-conductive material and might have accelerated bone formation. Applying the newly developed technique with artificiallysynthesized bone substitutes could be a useful method to reduce patientsu2019 stress. This newly established sinus floor elevation maybring better long term prognosis.

Subsequently, the TCP plate was inserted and supported with dental implants or micro-screws.Then, CBCT scan was performed to estimate temporal changes of the bone volume before surgery and at the time points of one, two,and five years after the surgery. Digital Imaging and Communication in Medicine (DICOM) files of CBCT were converted intoStereolithography (STL) files, using dedicated image analysis software for bone volume measurement and superimposing image datawith the best fit method. The difference of the image data representing the bone volume before and after the surgery was amalyzedthree-dimensionally, and the rate of change was calculated. This clinical research was approved by Ethics Committee, School ofDentistry, Iwate Medical University, and the patient's consent was also obtained. Ten cases of the sinus floor elevation with plate-shaped beta-TCPs have been performed since 2011. This study consisted of threemen and seven women. The average age of all the patients was 53.3 years old. Bone volume decreased slightly until 2 years after thesurgery and then increased again until 5 years after the surgery. There had no complications concerning the abnormalities of themaxillary sinus since the postoperative to the present, and it passed well. In most cases, newly formed bone in the sinus wasobserved on X-ray photos 6 month after the operation. The good stability of the dental implant, at the second stage surgery, wasconfirmed. TCP plates remained in the same position and achieved the secure space making for long time. Panoramic X-ray andX-ray CT analyses revealed that TCP-plates were resorbed in 5 years post operation in most cases. The results of our study demonstrated that it is possible to evaluate the change of bone volume of the sinus floor elevationquantitatively using the data of the STL form which converted DICOM. In this clinical trial, a new bone formation was observed in theempty space between the TCP plate and the original sinus floor. TCP is a bone-conductive material and might have accelerated boneformation. This newly established method for sinus floor elevation could bring better long-term prognosis.

Available in PDF, EPUB and Kindle. Book excerpt: While an oral surgery resident in training at the University of Michigan in 1977, I observed a strange phenomenon in a maxillary fracture patient. A unilateral ossification of the maxillary antrum occurred following a Le Fort III fracture that had been treated the year before. I wondered how trauma around the antrum due to an impacted maxilla and the inferiorly displaced orbital rim could lead to an ossified maxillary sinus. The trauma was bilateral, yet only one side responded by ossification. Furthermore, there appeared to be no functional impairment of the sinus. This unusual finding remained a mystery to me for many years. It wasn't until 1980, when Phil Boyne published the first paper on the sinus augmentation graft, that the mystery began to be solved. He found that preservation of the elevated sinus membrane created a confined space in which bone had the potential to form. Dr Boyne went on to show, in a primate study, that bone would readily form in the sinus floor, but that the newly formed bone would completely resorb over a period of a year unless dental implants were placed to help maintain the bone. Many other workers began to experiment with various bone-grafting materials, both in animals and clinically. Remarkably, most of the materials demonstrated new bone formation in the graft sites. Because dental implants were generally used as the measure of bone graft success and since there were very few implant failures reported in the literature, confusion resulted as to which grafting material to use. It seemed they all worked. To sort this out, a few patients with 5 to 7 mm of available bone were treated with sinus elevation and Gore-Tex over the lateral osteotomy site only. Despite no graft material be used, bone formed. As long as the sinus membrane remained intact, bone formed beneath the sinus membrane that had been tented up by simultaneously placed implants. The need to use any graft material at all appeared to be brought into question. At the same time, practitioners lined up in two camps: those who placed implants in delayed fashion after graft maturation and those who preferred simultaneous implant placement. Both approaches published a high level of success seeming to substantiate their protocols, but once again the measure of success was hampered by the lack of any human evidence for efficacy. A 5-year implant retrieval study was undertaken using standard titanium mini-implants that had been placed simultaneously with grafting. Interestingly, bone did not form well around these implants in this setting despite positive reviews in animal studies. Could we conclude that the delayed approach was preferred, or was there another factor, such as the implant surface, that could be important? Hydroxyapatite-coated, titanium plasma-sprayed, and acid-etched titanium implant surfaces seemed to do well in the grafted bone. Could this be the answer for implants placed into sinus bone grafts? As these developments are progressing, new interest is generated in jump-starting the natural inductive processes of bone formation through pharmacokinetics. Early results of human studies now appear to be favorable for using bone morphogenetic protein as a graft alternative that stands to modify all of the protocols in use to date. How these various approaches work toward definitive answers to the basic biologic and clinical questions is what this book is all about. The authors attempt to answer the how, why, when, and where of sinus grafting. It is hoped that the state of the art of sinus grafting will be elucidate for the reader in the ongoing quest for scientific knowledge and its clinical appilcation that will ultimately lead to excellence in patient care.
