Book excerpt: CONTEXTE ET OBJECTIFS L'allaitement maternel au-delà de 6 mois est aujourd'hui une pratique marginale qui se développe depuis plusieurs années, mais qui reste peu valorisée par les professionnels de santé. L'objectif de cette étude était d'approcher les logiques de choix et déterminants sociaux et familiaux d'un allaitement long, et d'élaborer des hypothèses sur le sens possible pour les mères de cette pratique. METHODE Une enquête par entretiens semi-dirigés a été menée auprès de 22 mères allaitant depuis plus de 6 mois. Les critères d'inclusion étaient d'avoir réalisé un allaitement maternel strictement supérieur à 6 mois, toujours en cours ou stoppé depuis moins de deux ans, exclusif ou non. Le critère de non-inclusion était l'appartenance à une culture non européenne. Une analyse thématique transversale a ensuite été réalisée. RESULTATS Trois profils de pratique d'allaitement maternel ont été mis en évidence: 1/L'allaitement « relationnel » est un moyen d'exprimer leur conception d'éducation par un maternage dit proximal, pour des femmes en interaction exclusive avec l'enfant. Forme d'affirmation de soi contre leurs modèles familiaux ou les normes sociales, et fonction de réparation d'évènements traumatisants ; 2/L'allaitement « naturel » s'inscrit dans un mode de vie « naturel » , des préoccupations environnementales et des pratiques alternatives. Il est une évidence pour la santé de l'enfant et de la mère en opposition par cette pratique à l'industrie alimentaire ; 3/L'allaitement « logique » est abordé dans une démarche logique et pratique, sans théorie ni idéologie sur la question. Acte nourricier et geste universel, allant de soi dans la logique de leur grossesse. Certaines mères appartiennent à un seul profil et pour d'autres elles associent des caractéristiques communes à plusieurs profils. La plupart soulignent l'absence de transmission familiale de l'allaitement maternel. Pour certaines mères le sevrage est mal vécu et nécessite une rupture du lien fusionnel inscrivant l'allaitement long dans une modalité anti-dépressive, et parfois idéalisé en sevrage « naturel » choisi par l'enfant. La crainte de la sexualisation et d'une tonalité incestueuse de l'allaitement est souvent exprimée. Pratique parfois dissimulée, presque clandestine à l'abri des regards et de la pression normative de la société, beaucoup cachent leur allaitement long au médecin. Ces mères allaitant au-delà de 6 mois font plus confiance aux réseaux sociaux et associatifs qu'au corps médical jugé trop effacé ou trop interventionniste. DISCUSSION Le médecin a peu de place dans l'accompagnement de ces femmes qui allaitent longtemps. Une attitude médicale empathique à leur égard pourrait les conduire à redonner leur confiance aux soignants pour que leur pratique d'allaitement sorte d'une certaine forme de clandestinité.

Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.

Yet in 1992-93, some five hundred people died from cholera in the Orinoco Delta of eastern Venezuela. In some communities, a third of the adults died in a single night, as anthropologist Charles Briggs and Clara Mantini-Briggs, a Venezuelan public health physician, reveal in their frontline report. Why, they ask in this moving and thought-provoking account, did so many die near the end of the twentieth century from a bacterial infection associated with the premodern past? It was evident that the number of deaths resulted not only from inadequacies in medical services but also from the failure of public health officials to inform residents that cholera was likely to arrive. Less evident were the ways that scientists, officials, and politicians connected representations of infectious diseases with images of social inequality. In Venezuela, cholera was racialized as officials used anthropological notions of "culture" in deflecting blame away from their institutions and onto the victims themselves. The disease, the space of the Orinoco Delta, and the "indigenous ethnic group" who suffered cholera all came to seem somehow synonymous. One of the major threats to people's health worldwide is this deadly cycle of passing the blame. Carefully documenting how stigma, stories, and statistics circulate across borders, this first-rate ethnography demonstrates that the process undermines all the efforts of physicians and public health officials and at the same time contributes catastrophically to epidemics not only of cholera but also of tuberculosis, malaria, AIDS, and other killers. The authors have harnessed their own outrage over what took place during the epidemic and its aftermath in order to make clear the political and human stakes involved in the circulation of narratives, resources, and germs.
