Book excerpt: Contexte : Le trouble lié à l'usage de l’alcool (TLUA) est très répandu dans la population générale à travers le monde, touchant 3.6 % de la population âgée de 15 à 64 ans. De plus, le TDA/H est surreprésenté chez les sujets atteints de troubles liés à l’usage de substances (TLUS). Méthodologie : Il s’agit d’une étude observationnelle transversale et mono centrique. Ont été inclus les patients hospitalisés entre 2015 et 2019 dans un service d’hospitalisation d’addictologie pour un sevrage alcoolique seul ou associé à d’autres substances pour qui une évaluation TDA/H (ASRS et WURS) était disponible (n=226). Objectifs : L’objectif de notre étude est d’étudier plus finement le lien existant entre ces deux phénotypes complexes que sont le TDA/H et le TLUA. Résultats : La prévalence du TDA/H dans l’enfance (échelle WURS) est de 28.8 % (n=65). La prévalence du TDA/H à l’âge adulte sur les 6 derniers mois (échelle ASRS v1.1) est de 29.2% (n=66). Les sujets dépistés avec un TDA/H dansl’enfance sont hospitalisés en moyenne pour plus de produits. Ces patients remplissaient en moyenne plus de critères DSM-V pour le TLUA. L’analyse factorielle exploratoire retrouve une solution à 3 facteurs pour le TLUA. Le facteur « conséquences négatives » est associé à la positivité de la WURS et à la dimension « impulsivité » de cette échelle. Conclusion : On retrouve une surreprésentation du TDA/H parmi notre échantillon souffrant d’un TLUA. Cela nous a permis de retrouver une association entre la dimension impulsivité de l’échelle WURS et le fait de plus souffrir des conséquences négatives de l’alcool, ce qui a déjà été décrit dans la littérature.

Book excerpt: Contexte : Au niveau mondial, la prévalence du Trouble Déficit de l'Attention et Hyperactivité (TDAH) de l'adulte est de 3.4%. Chez l'enfant ce trouble est un facteur de risque important de développer un trouble de l'usage de l'alcool (TUA). Peu diagnostiqué chez l'adulte, il est pourtant associé avec un risque de développer un trouble des substances. Méthode : Nous avons réalisé une revue systématique de la littérature à partir des bases de données Medline et Science direct selon le pattern de mots clés suivant : « ADHD » AND « alcohol » AND « adult » AND « epidemiology ». Nous avons ensuite sélectionné les articles selon des critères prédéfinis. Résultats : 22 articles pertinents ont été sélectionnés. La prévalence du TDAH à l'âge adulte chez les patients alcoolo dépendants était de 7.7% à 32.9%. Chez les patients adultes TDAH, la prévalence d'abus d'alcool était de 23.3% à 47% et celle de l'alcoolo dépendance de 20.8 %. Dans la majorité des études, on retrouve une association statistiquement significative entre TDAH de l'adulte et TUA. Les complications dues à la consommation d'alcool sont plus fréquentes chez les patients adultes TDAH. Conclusion : Il semble utile de repérer les patients TDAH dans les services d'addictologie ou en médecine générale. Cela permettrait d'adapter la prise en charge, ou de prévenir le passage d'un usage simple à un mésusage.

Nous avons choisi cette approche pour décrire les liens unissant le trouble déficit attentionnel / hyperactivité (TDAH) infantile à l'alcoolo-dépendance à l'âge adulte par une analyse de la littérature. Les données épidémiologiques montrent une association du TDAH, dans ses dormes persistantes, sévères et associées à des conduites antisociales précoces, à une plus grande fréquence d'abus et de dépendance à l'alcool à l'âge adulte. L'alcoolisme est alors plus précoce et plus intense. Il est souvent comorbide à une personnalité antisociale et intégré dans une polytoxicomanie. Les approches tempéramentales, cognitives, génétiques et neurobiologiques aident à circonscrire le lien entre les deux cadres morbides. Ces troubles partagent une vulnérabilité familiale sous-tendue par des facteurs génétiques. Des niveaux élevés d'activité, de recherche de sensation et de nouveauté, et certains traits cognitifs relevant d'un déficit préfrontal, sont les témoins précoces de cette susceptibilité. Ces différents traits sont cohérents avec les hypothèses neurobiologiques, en faveur d'une dysfonction dopaminergique impliquée dans un déficit au renforcement différé. Ce déficit du système de récompense pourrait prédisposer à l'usage précoce de substances, à la survenue et à la pérennisation d'une dépendance à l'alcool. Les différents facteurs de vulnérabilité sont alors à concevoir en interaction avec l'environnement tout le long du développement. Trois vignettes cliniques permettent d'illustrer les différentes étapes développementales (enfance, adolescence, âge adulte) et de souligner l'hétérogénéité clinique associée aux premières alcoolisations

Book excerpt: Recent years have seen important developments in the computer and game industry, including the emergence of the concept of serious games. It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools? What are the possible links between such tools and social networks? Is there a gap between evidence-based results and market development? Are there any quality challenges? What future developments and studies are needed in the field?
