Ces molécules, représentées en premier lieu par le dabigatran, le rivaroxaban et l'apixaban, ont d'abord été cantonnées à la seule indication de prévention des événements thromboemboliques veineux après prothèse totale de hanche ou de genou. Les laboratoires ont par la suite fait des demandes d'extension d'indication, avec des fortunes diverses. Certaines sont en cours d'examen par les agences du médicament, et, parallèlement, de nouvelles molécules sont en cours de développement. L'anticoagulation est un paramètre physiologique intervenant dans de nombreuses pathologies, et la population-cible augmente. Sa maîtrise et sa justesse sont essentielles : trop faible et le traitement sera inefficace, trop forte et le patient sera sujet à des hémorragies ; les conséquences pour le patient pouvant être graves dans les deux cas de figure. A l'heure des entretiens pharmaceutiques, le pharmacien d'officine a un rôle d'accompagnement auprès des patients afin de s'assurer d'une bonne observance et d'une efficacité optimale du traitement.

Book excerpt: Contexte : La fibrillation atriale du sujet de plus de 75 ans est une pathologie fréquente nécessitant une anticoagulation efficace. Jusqu’à peu, les anticoagulants oraux n’étaient représentés que par les AVK que de nombreux médecins hésitaient encore à prescrire chez les sujets âgés, leur préférant souvent un antiagrégant plaquettaire. Depuis fin 2011, deux nouveaux anticoagulants oraux, Pradaxa et Xarelto ont obtenu l’AMM dans le traitement de la fibrillation atriale, source d’une possible évolution des pratiques médicales. Objectifs : Dresser un bilan des pratiques actuelles des généralistes bretons concernant l'anticoagulation des sujets de plus de 75 ans en fibrillation atriale non valvulaire. Envisager les perspectives d’évolution thérapeutique pour les patients déjà sous AVK ou Kardégic, suite à l’arrivée des nouveaux anticoagulants oraux (NACO). Méthode : Etude quantitative de 317 questionnaires auto administrés par un échantillon cible de généralistes bretons Résultats : 72,3% des généralistes bretons avaient encore des patients de plus de 75 ans en fibrillation atriale traités uniquement par Kardégic, mais seuls 50,9% d’entre eux envisageaient de le remplacer par un NACO. Le Previscan restait l’AVK utilisé préférentiellement par 79,7% des généralistes bretons au détriment de la Coumadine. Chez les patients bien équilibrés sous AVK, 31,1% des médecins jugeaient utile un relais AVK/NACO, mais seuls 13,8% envisageaient de remplacer systématiquement les AVK par les NACO. Chez les sujets mal équilibrés sous AVK, 89,4% envisageaient de remplacer les AVK par les NACO. Conclusion : Trop de patients de plus de 75 ans en fibrillation atriale n’étaient toujours pas anticoagulés et le Previscan restait trop prescrit au détriment de la Coumadine. Le remplacement du Kardégic par les NACO divisait l’opinion et la grande majorité n’était pas en faveur d’un relais systématique AVK/NACO chez les sujets bien équilibrés sous AVK. En revanche, un relais AVK/NACO chez les sujets mal équilibrés sous AVK faisait consensus.

Book excerpt: Les anticoagulants, antivitamines K et héparines sont sur le marché depuis plusieurs dizaines d’années et sont devenus des références dans le traitement préventif et curatif des maladies thromboemboliques. Néanmoins, leurs effets secondaires et leurs inconvénients d’utilisation ont poussé l’industrie pharmaceutique à rechercher un anticoagulant idéal. Trois molécules, utilisables par voie orale, ont donc fait récemment leur apparition sur le marché : 2 inhibiteurs du facteur Xa (le rivaroxaban et l’apixaban) et un inhibiteur de la thrombine (le dabigatran), le facteur Xa et la thrombine étant des cibles idéales pour un traitement anticoagulant. Cependant, plusieurs questions se sont rapidement posées, notamment concernant le risque hémorragique, puisque ce sont des molécules qui ne possèdent pas d’antidote et pour lesquels les tests biologiques usuels ne sont pas utilisables. Dans ce travail, nous avons évalué leurs conditions de prescription au travers d’une étude de 12 cas de patients hospitalisés et traités par ces nouveaux anticoagulants oraux. Cette étude a permis de rendre compte du profil des patients traités par ces nouvelles molécules et d’évaluer si les recommandations et les nombreux rappels de l’ANSM ont été suivis par les prescripteurs. Ce travail démontre clairement la nécessité d’encadrer l’utilisation de ces molécules et de suivre les recommandations, notamment concernant leurs indications, leurs posologies et adaptations posologiques, leurs contre-indications et précautions d’emploi.
