Book excerpt: Ce travail avait pour objectif de déterminer et de comprendre les motivations professionnelles et personnelles des sages-femmes exerçant en maison de naissance. Huit entretiens ont été réalisés avec des sages-femmes travaillant dans six maisons de naissance françaises en expérimentation. Cette étude a permis de mettre en évidence que l'accompagnement global fait partie de l'une des motivations existantes cependant ce n'est pas la principale. En effet, les sages-femmes qui exercent n'ont pas attendu le développement des maisons de naissance en France pour suivre les couples en accompagnement global. Les sages-femmes apprécient l'autonomie et l'enrichissement que leur procurent les maisons de naissance ainsi que le travail en équipe, la sécurité et l'utilisation de toutes leurs compétences et leurs responsabilités de sages-femmes. Cependant, l'expérimentation des maisons de naissance demande un travail administratif supplémentaire de gestion ce qui est épuisant pour ces sages-femmes et la non-reconnaissance financière de leur travail est un frein majeur à ce type d'exercice.

Book excerpt: But : L'objectif de ce mémoire était de décrire les freins et les motivations à la création et à l'exercice en Maison de Naissance des sages-femmes libérales de la Savoie. Matériel et méthodes : Il s'agit d'une étude qualitative descriptive prospective monocentrique réalisée par entretiens semi-directifs, à l'aide d'un guide d'entretien, auprès de 10 sages-femmes libérales exerçant en Savoie. Nous avons réalisé une analyse thématique horizontale à partir de la retranscription intégrale des entretiens. Résultats : L'analyse a montré que l'implantation d'une maison de naissance n'est envisageable et viable que si les sages-femmes se considèrent comme faisant partie d'un réseau pluri professionnel en adéquation avec le fonctionnement des maisons de naissance et les compétences des sages-femmes. Le statut de la sage-femme semble valorisant mais la reconnaissance de l'accompagnement global d'un point de vue financier apparaît comme nécessaire. Les conditions de travail peuvent être améliorées notamment grâce au travail en équipe au sein de la structure pour palier à la disponibilité importante de la sage-femme. Enfin, la maison de naissance tend à être considérée comme une pluralité de l'offre périnatale, et non plus comme une alternative, la demande des patientes étant largement supérieure à l'offre proposée par les sages-femmes. Conclusion : Si les sages-femmes interrogées pensent qu'une maison de naissance est légitime sur le département de la Savoie, le sujet du lieu pose la question de viabilité, entre maternités à consulter et réseau à fédérer.

Book excerpt: Institutionalizing Gender analyzes the relationship between class, gender, and psychiatry in France from 1789 to 1900, an era noteworthy for the creation of the psychiatric profession, the development of a national asylum system, and the spread of bourgeois gender values. Asylum doctors in nineteenth-century France promoted the notion that manliness was synonymous with rationality, using this "fact" to pathologize non-normative behaviors and confine people who did not embody mainstream gender expectations to asylums. And yet, this gendering of rationality also had the power to upset prevailing dynamics between men and women. Jessie Hewitt argues that the ways that doctors used dominant gender values to find "cures" for madness inadvertently undermined both medical and masculine power—in large part because the performance of gender, as a pathway to health, had to be taught; it was not inherent. Institutionalizing Gender examines a series of controversies and clinical contexts where doctors' ideas about gender and class simultaneously legitimated authority and revealed unexpected opportunities for resistance.
