En effet, notre étude prospective et descriptice de 6 mois au centre d'orthogénie de Tours s'est intéressée aux femmes demandant une IVG dans l'année suivant leur dernier accouchement. Les résultats, obtenus à partir de 60 femmes, ont révélé les difficultés de la prise d'une contraception, notamment avec les microprogestatifs. Afin de mieux préparer les femmes à la reprise d'une contraception en post-partum, il serait alors judicieux de dégager un moment consacré à ce sujet au cours de la grossesse.

Le code d'Hammaerobi (XVnème siècle avant JC) le sanctionne, le serment d'Hippocrate (IVème siècle avant JC) l'interdit également: "(H.) je ne remettrai a aucune femme un pessaire abortif." Jusqu'au 26 Novembre 1974, jour où Simone VEIL prononçait devant l'assemblée le discours qui allait mener à la législation sur l'avortement. Loi, qui depuis a été modifiée, notamment par Mme Aubry, donnant ainsi naissance en 2001 à la loi permettant l'avortement jusqu'à 14 semaines d'aménorrhées et supprime l'obligation d'autorisation parentale pour les mineurs. Cependant, de nos jours environ 5% des interruptions volontaires de grossesse surviennent dans le post-partum malgré le développement des méthodes contraceptives telle que par exemple Cérazette®, Microval® ou l'implant et de la contraception d'urgence. Qu'est ce qui peut expliquer ce taux d'interruptions volontaires de grossesse dans le post-partum? La prévention de ces grossesses non désirées est- elle faite de manière optimale? Une étude quantitative réalisée auprès de femmes réalisant une interruption volontaire de grossesse dans l'année suivant un accouchement permet d'apporter des éléments de réponse à ces questions. Leur analyse conduira à des propositions pour améliorer la prévention de ces grossesses non désirées. Une réelle modification des conditions d'information des femmes sur la contraception du post-partum est nécessaire, afin que celles-ci intègrent réellement la contraception du post-partum dans leur vie et comprenne le risque si elles n'utilisent pas celle-ci. La sage-femme a un rôle primordial dans la prévention de ces grossesses non désirées.

Egalement la contraception proposée aux femmes dans le post-partum doit être plus adaptée à leur demande et il faut pour cela changer nos pratiques.

Elle formule enfin également des propositions dans le but d’améliorer localement l’accès à l’information sur la contraception classique et d’urgence mais aussi la prévention de ces IVG.

Book excerpt: En dépit d'une diffusion et d'une utilisation massives de méthodes de contraception médicale, le taux de grossesses non prévues reste élevé, et l'on estime à 5% les IVG qui ont lieu dans les six mois suivant un accouchement, ce qui représente environ 11000 femmes par an en France concernées. Mieux connaitre et comprendre les raisons de la survenue d'une nouvelle grossesse dans les six mois après un accouchement. Améliorer la prise en charge des femmes en post-partum. Etude qualitative par entretiens semi-dirigés chez douze femmes majeures ayant découvert une nouvelle grossesse dans les six mois après un accouchement, et rencontrées d'octobre à décembre 2016. La survenue d'une grossesse rapprochée est due à l'échec ou à l'absence de contraceptif. Différents déterminants interviennent dans la décision d'interrompre ou de mener cette grossesse à terme. Le conjoint et l'entourage influencent le choix contraceptif. Il n'y a pas un moment unique idéal pour parler de la contraception du post-partum. Les représentations erronées et les idées reçues constituent des freins majeurs au choix d'une méthode contraceptive. Discussion : Il semble nécessaire d'améliorer l'information contraceptive donnée au couple ainsi qu'aux professionnels de santé. Lutter contre les idées reçues et les représentations améliorerait l'adhésion à une méthode contraceptive. Aborder le sujet de la contraception à différents moments du péri-partum permettrait une meilleure connaissance des méthodes contraceptives possibles après l'accouchement et tout au long de la période de fécondité d'une femme. La contraception du post-partum nécessite la délivrance d'une information répétée dans le temps par les professionnels de santé. Elle doit s'adapter aux trajectoires de vie de la patiente et du couple et s'intégrer dans ce que l'on appelle la planification familiale du post-partum.
