Book excerpt: L'utilisation des antirétroviraux a permis de nombreuses améliorations du système immunitaire des patients infectés par le VIH qui se sont traduites par une diminution de la morbidité et de la mortalité liées à l'infection par le VIH. Toutefois, ces traitements présentent certaines insuffisances (restauration incomplète, toxicité sévère, augmentation de la charge virale plasmatique à l'arrêt des traitements...). L'utilisation de cytokines immunostimulantes, dont l'Interleukine-2, en complément des traitements antirétroviraux, a donc pour objectif de pallier ces insuffisances. D'un point de vue immunologique, l'Interleukine-2 a permis une augmentation importante et durable des taux de lymphocytes T CD4 chez tous les patients infectés par le VIH, y compris chez les patients en «échec immunologique des traitements» dont les taux de lymphocytes T CD4 restent inférieurs à 200/millimètres cube. Ces lymphocytes sont fonctionnels et semblent avoir plusieurs origines. D'un point de vue virologique, l'utilisation d'Interleukine-2 n'entraîne pas d'augmentation de la charge virale plasmatique chez les patients, contrairement aux craintes passées. Elle semble même parfois diminuer celle-ci par rapport au traitement antirétroviral ; mais elle ne permet pas d'éradiquer le virus chez les patients infectés. Aujourd'hui, l'utilisation de l'Interleukine-2 se fait par injection en voie sous-cutanée de doses moyennes (autour de 9 MUI/j) permettant d'obtenir une diminution des effets secondaires. Toutefois, des effets indésirables fréquents existent encore lors de son utilisation. Mais ils sont de faible grade et disparaissent après les cures d'Interleukines-2. En définitive, même si le bénéfice clinique de l'immunothérapie par l'Interleukine-2 n'est pas connu, les résultats déjà obtenus chez les patient infectés par le VIH et surtout chez les "non-répondeurs" démontrent que l'Interleukines-2 a une place dans l'arsenal thérapeutique utilisé contre l'infection à VIH.

In following this exciting story, this report looks at the practical consequences of these developments for pharmacovigilance practitioners. The report provides a comprehensive resource for those considering how to strengthen their pharmacovigilance systems and practices, and to give practical advice. But the report does not specify instant solutions. These will inevitably be situation specific and require careful consideration taking into account local needs. However, the CIOMS Working Group VIII is convinced that the combination of methods and a clear policy on the management of signals will strengthen current systems. Finally, in looking ahead, the report anticipates a number of ongoing developments, including techniques with wider applicability to other data forms than individual case reports. The ultimate test for pharmacovigilance systems is the demonstration of public health benefit and it is this test which signal detection methodologies need to meet if the expectations of all stakeholders are to be fulfilled.

Biohydrogen is considered a promising future biofuel because of its intrinsic clean and high-energy content properties and the way it is produced. In addition to being produced through environmentally friendly biological means, its conversion to energy yields only pure water, which is an ideal energy carrier in reducing greenhouse gas emissions from fossil fuel combustion. Unlike other well-developed biofuels such as bioethanol and biodiesel, biohydrogen production is still in the early stage of development. A variety of technologies are being developed for biohydrogen production. This chapter presents a review of the state-of-the-art and perspectives of bioprocess design for biohydrogen production research in the context of pathways, microorganisms, metabolic flux analysis, process design, and reactor system. Challenges and prospects of biohydrogen production are also outlined.
