Un relai à domicile par une sage femme libérale ou de PMI est donc fréquemment mis en place pour compléter l'accompagnement post- natal. L'accompagnement au retour à domicile permet-il de répondre aux attentes des patientes ? Les patientes ayant un suivi à domicile organisé ont été recrutées en suite de couche. Un questionnaire leur a ensuite été envoyé un mois après par e-mail. Elles ont alors été interrogées sur leur conditions de vie, le déroulement de leur grossesse, de l'accouchement et du séjour à la maternité ; puis sur leurs besoins et leurs difficultés au retour à domicile, sur le suivi à domicile et leurs attentes ainsi que sur les difficultés persistantes à plus long terme. 153 patientes ont répondu au questionnaire. Dans 89 % des cas, l'accompagnement au retour à domicile a permis de répondre (plutôt ou tout à fait) aux attentes des patientes. On constate une forte évolution de l'accompagnement au retour à domicile notamment par les sages-femmes libérales (88 % d'après notre étude contre 20 % d'après l 'EPN de 2010). Pour conclure, l'accompagnement au retour à domicile permet globalement de répondre aux attentes des patientes. Nous avons tout de même pu voir que des perspectives d'amélioration étaient possibles.

La diminution de la durée des séjours en maternité a placé la sage-femme libérale au coeur du suivi postnatal. De plus, l'Entretien Postnatal Précoce, recommandé par la Haute Autorité de Santé depuis 2014, revient dans l'actualité par sa mise en place de manière obligatoire en 2022. L'objectif de l'étude est de faire un état des lieux des attentes des nouvelles accouchées en matière d'accompagnement postnatal. Méthodes : Il s'agissait d'une étude épidémiologique observationnelle qualitative, par entretiens semi-directifs auprès de 14 femmes ayant accouché en Moselle en 2021. Les entretiens ont été réalisés en visioconférence sur la base du volontariat après diffusion d'un flyer de recrutement. Le critère de jugement principal ciblait les attentes les attentes des nouvelles accouchées dans le post-partum. Résultats : Les entretiens ont montré que la majorité des difficultés ressenties par les femmes étaient psychologiques, quelle que soit la période du post-partum en cours. Les problématiques ressenties étaient centrées sur elles-mêmes, leur(s) enfant(s) ou leur couple. L'accompagnement prodigué par la sage-femme libérale à domicile, malgré un sentiment de satisfaction global, a été jugé comme insuffisant par les patientes et trop peu axé sur le plan psychologique. L'Entretien Postnatal Précoce était quant à lui méconnu de la plupart des accouchées, mais souhaité par 100% d'entre-elles. Conclusion : Cette étude a permis de faire le point sur les attentes des accouchées en matière d'accompagnement postnatal en mettant en lumière les grandes problématiques qui, pour les femmes nécessitent un suivi plus structuré de leur post-partum. La sage-femme libérale, au coeur de ce suivi, reste l'interlocuteur de prédilection auquel les femmes ont recours durant leur post-partum.
