Book excerpt: Notre thèse porte sur le pronostic du cancer du sein bilatéral. 3634 cas de cancers du sein ont été étudiés de 1977 à 2001 au centre de lutte contre le cancer de Nice (Centre Antoine Lacassagne CAL Nice) dont 3336 unilatéraux et 298 bilatéraux. Parmi ces 298 cas de cancers du sein bilatéraux, 103 étaient synchrones et 195 asynchrones, ce qui est conforme à la littérature. L'étude statistique montre qu'il n'y a pas de différences pronostiques entre les cancers du sein unilatéraux et les cancers du sein bilatéraux. En revanche, il semble que les cancers bilatéraux asynchrones ont un meilleur pronostic que les cancers unilatéraux, alors que les cancers bilatéraux synchrones ont un pronostic plus défavorable que les cancers unilatéraux ; cependant, la différence d'effectifs des populations étudiées ne permet pas de conclusion statistique. Le meilleur pronotic des cancers asynchrones par rapport aux cancers synchrones est bien mis en évidence. Les éléments pronostiques de consensus du cancer du sein, en général, ont une influence variable sur la différence de pronostic des cancers synchrones par rapport aux cancers asynchrones. En effet, si la ménopause n'a pas d'influence particulière sur la différence de pronostic des cancers synchrones par rapport aux cancers asynchrones, les récepteurs hormonaux ont une valeur pronostique, en particulier quand ils sont associés au paramètre synchrone/asynchrone. Par ailleurs, même si l'envahissement ganglionnaire histologique est un facteur pronostique prépondérant, le caractère synchrone garde, toutefois, un poids pronostique en lui-même. Le caractère synchrone semble dont être un facteur pronostique péjoratif indépendant des autres critères. Ainsi, peut-être, devrions-nous en tenir compte dans notre démarche thérapeutique et dans notre réponse à l'attente des patients.
