Avec plus de 16000 décès par an lié au cancer colorectal en France, le cancer du rectum constitue un véritable problème de santé publique. La chirurgie reste l’élément central de la prise en charge du cancer du rectum. Pourtant, elle s’est considérablement modifiée depuis une vingtaine d’années. Avec les travaux de Heald, dans les années 80, l’exérèse du mésorectum est devenue la règle. Les principes de la chirurgie carcinologique du rectum et ses critères de qualité font l’objet de recommandations actualisées en 2007. De nombreuses avancées ont par ailleurs été réalisées dans les traitements associés à la chirurgie. Ainsi, les traitements néoadjuvants, par radio ou radiochimiothérapie, réduisent le taux de récidive locale et augmentent, grâce à une réduction du volume tumoral, le taux de conservation sphinctérienne. A la lumière de ces évolutions, il nous est apparu nécessaire d’étudier non seulement la prise en charge chirurgicale des cancers du rectum au CHU de Reims mais aussi les facteurs pronostiques de cette pathologie.
