Book excerpt: La démographie médicale évoluant, il convenait de s'interroger sur l'implication actuelle du médecin généraliste dans le suivi des femmes enceintes. Ce dernier connaît la patiente, sa situation socioprofessionnelle, familiale et ses antécédents personnels. Ce statut lui permet de gérer les angoisses et imprévus engendrés par une grossesse, en s'adaptant aux particularités des patientes. Nous nous sommes focalisés sur le suivi du premier trimestre des grossesses à bas risques, afin de représenter un grand nombre de généralistes, sans prétendre gérer les risques qui restent du ressort du spécialiste. L'étude vise à établir un état des lieux de ce suivi. Le généraliste s'implique-t-il, de quelle manière, selon les recommandations ? Souhaite-t-il s'engager davantage et attend-il des aides ? Autant de questions posées, via un questionnaire, aux médecins exerçant à Colombes, Gennevilliers et Villeneuve-la-Garenne. 90 ayant répondu dans le cadre d'un entretien ou après remise des feuillets en mains propres, le taux de retour obtenu fut de 91 %. L'analyse révèle que 76 % des répondants suivent des femmes enceintes au premier trimestre de grossesse, mais surtout que 89 % souhaitent éventuellement suivre leurs patientes enceintes. Ce désir d'implication est d'autant plus intéressant que les médecins souhaitent participer à des informations sur ce sujet. 90 % d'entre eux attendent principalement un contact facilité avec un obstétricien. Le réseau périnatalité ville - hôpital recueille moins de suffrage, compte tenu de l'investissement en temps requis. On peut déplorer que seuls 9 % des généralistes appliquent totalement les recommandations (prescriptions biologiques, échographiques, de l'HT21 et déclaration administrative), d'où l'élaboration d'un guide de bonnes pratiques. Au vu de cette étude, l'implication des généralistes dans le suivi des femmes enceintes est incontournable, compte tenu de l'actuelle démographie médicale et de leur réel désir d'engagement. Moyennant une formation simple et adaptée sur les pratiques recommandées, ce suivi pourrait fortement se développer

Book excerpt: Contexte : En 2014, l'hôpital Jeanne de Flandre a créé une consultation dite d'orientation pour proposer le suivi le plus adapté aux femmes enceintes. L'objectif de l'étude est de recueillir le ressenti des femmes enceintes sur le vécu de la consultation et du suivi ambulatoire afin d'améliorer notre pratique. Méthode : Une étude qualitative par entretiens semi dirigés auprès de femmes enceintes à bas risque majeur ayant bénéficié d'une consultation d'orientation à l'hôpital Jeanne de Flandre. Résultat : 15 entretiens ont été réalisés. La consultation d'orientation a été bien vécue par les patientes. Les femmes enceintes déclarent un manque d'information sur le suivi de grossesse. Cette consultation permet de s'informer sur le suivi de grossesse, de se sentir en sécurité et d'être rassurée. Au final, cette consultation permet de se préparer à la suite du suivi. Les critiques sur la consultation sont le caractère redondant en cas de grossesse précédente, le trop plein d'information, la survenue tardive dans le suivi, le faible choix dans les professionnels libéraux. Suite à cette consultation, elles acceptent le principe d'un suivi ambulatoire. Elles reconnaissent ne pas avoir besoin d'un suivi plus important. Le suivi ambulatoire a été globalement bien vécu par les patientes. Elles en apprécient la proximité, l'intimité, la flexibilité. Au final ce suivi ambulatoire apparaît équivalent au suivi hospitalier et leur parait également plus adapté avec le recul. Enfin, les patientes ont relevé peu d'échanges entre professionnels de la périnatalité mais notent que ces échanges se font correctement avec le dossier obstétrical. Conclusion : La consultation d'orientation permet de redonner un sens au caractère physiologique de la grossesse. Le suivi ambulatoire est accepté et apprécié des patientes. La création de cette consultation devrait ainsi permettre un retour du suivi de femmes enceintes vers la ville et la médecine ambulatoire.

La mise en oeuvre de parcours de soins adaptés à ces niveaux de risque n'est pas aisée. Objectif : Dans une population de femmes suivies par un médecin spécialiste hospitalier, décrire la typologie observée (type « 1 ») des parcours de soins, que les grossesses soient à bas risque (M1, N1, O1) ou à haut risque (P1, Q1, R1), et proposer une typologie idéale (type « 2 ») pour les grossesses à bas risque (M2, N2, O2) et à haut risque (P2, Q2, R2). Evaluer les ressources médicales nécessaires à une typologie idéale du suivi des patientes. Méthode : Analyse des dossiers obstétricaux des patientes suivies en consultation prénatale par un spécialiste à l'Hôpital Jeanne de Flandre, pendant la semaine du 8 au 14 septembre 2015. Proposition d'une répartition « idéale » des parcours de soins des patientes selon leur typologie. Résultats : Parmi les 87 patientes incluses pendant la période d'étude, la moitié étaient à bas risque et l'autre à haut risque selon la classification HAS : 31 étaient classées A (36%), 11 A1 ou A2 (13%) et 45 B (51%). Selon notre typologie de parcours de soins, nous retrouvions parmi les patientes à bas risque 22 patientes M (25%), 9 patientes N (10%) et 11 O (13%) et parmi les patientes à haut risque 30 P (34%), 11 Q (13%) et 4 R (5%). Seules 21 patientes (24%) avaient un parcours de soins considéré comme « idéal » (type 2). Si l'ensemble des patientes avaient ce type idéal de parcours en type 2, 27% des créneaux seraient en théorie libérés sur l'ensemble de leurs suivis. Une fiche simplifiée de la classification HAS (mémo) est proposée dans le but de faciliter la prise initiale de rendez-vous des patientes. Conclusion : Dans notre maternité, l'utilisation d'une classification des parcours de soins permet théoriquement d'adapter le suivi des patientes à leur niveau de risque sans nécessiter de ressources médicales supplémentaires.

It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
