Book excerpt: Le remodelage ventriculaire gauche au cours de l’insuffisance cardiaque à fraction d’éjection ventriculaire gauche altérée est le phénomène définissant les modifications anatomiques, tissulaires et cellulaires au cours de l’évolution de la maladie. Il s’agit le plus souvent d’un remodelage associé à un pronostic péjoratif. Le remodelage inverse est moins fréquent et est associé à un pronostic plus favorable. Néanmoins, les données concernant ce phénomène sont peu nombreuses. Ce travail avait pour but de caractériser le remodelage inverse dans une cohorte monocentrique de 196 patients insuffisants cardiaques avec FE VG altérée, inclus entre 2008 et 2010 et ayant eu au moins deux échocardiographies espacées de 3 à 18 mois et un suivi clinique d’au moins 3 ans. Le remodelage inverse était défini par une diminution d’au moins 10% du diamètre télédiastolique VG et par une augmentation d’au moins 10 points de la FEVG. Un remodelage inverse a été observé chez 21 patients (10,7%). Les facteurs associés à sa survenue étaient l’obésité (OR=3,6), le caractère de novo de l’insuffisance cardiaque (OR=0,19) et une FE VG initiale plus basse (OR=0,95). En analyse multivariée le remodelage inverse avait par ailleurs un impact positif sur la survie sans transplantation cardiaque (HR = 0,22 IC95% 0,06-0.95, p=0,04). De façon paradoxale, l’obésité semble donc associée au remodelage inverse dans l’insuffisance cardiaque à FEVG altérée, ce qui nécessite confirmation dans d’autres cohortes et d’en comprendre les mécanismes. De façon plus attendue, ce remodelage inverse a également une valeur pronostique positive.

Book excerpt: L'objectif de ce travail était de mieux appréhender les mécanismes impliqués dans la réponse à la resynchronisation biventriculaire en insistant sur la caractérisation écho-cardiographique du substrat éligible à un traitement par resynchronisation cardiaque (TRC). Afin de valider dans notre cohorte les critères de réponse au TRC, nous avons évalué l'impact pronostic au long cours de l'amélioration du strain longitudinal du ventricule gauche (VG), de la fraction d'éjection VG et des volumes VG 6 à 9 mois après un TRC. L'amélioration du volume télésystolique du VG (remodelage inverse) semble être le marqueur le plus pertinent du pronostic au long cours après TRC. De plus, l'insuffisance mitrale secondaire significative persistante malgré un TRC semble être un facteur pronostic important indépendant du remodelage VG. Nous avons démontré qu'il existe un certain degré de désynchronisation mécanique (fondé sur la mesure des délais entre paroi) dans les insuffisances cardiaques à fraction d'éjection préservée qui est semblable aux insuffisances cardiaques à fraction d'éjection basse à QRS fins et supérieur aux témoins hypertendus. Cependant, les paramètres de désynchronisation échographiques considérés comme les substrats de la réponse au TRC (fondés sur la contraction type bloc de branche gauche) ne sont que très rarement retrouvés chez ces patients. Ces paramètres sont fréquemment présents dans les insuffisances cardiaques à fraction d'éjection basse avec un QRS large et une indication de TRC. Le strain longitudinal du septum interventriculaire permet de caractériser 3 patterns de contraction septale eux même prédicteurs du remodelage inverse, des hospitalisations pour insuffisance cardiaque, de la mortalité cardiovasculaire et de la mortalité toute cause après un TRC. L'élargissement de la largeur du QRS après un TRC est quant à elle un prédicteur de l'absence de remodelage inverse, et d'un excès de mortalité cardiovasculaire et toute cause après un TRC. L'association d'un pattern de contraction septal peu prédicteur de réponse au TRC et d'un élargissement du QRS après le TRC permet d'identifier des patients avec une probabilité de réponse très faible au TRC.

L'étude ProBioHF a pour objectif d'identifier des marqueurs biologiques pronostics de l'évolution de l'insuffisance cardiaque après la resynchronisation. Les données cliniques de cette étude sont analysées pour quantifier le phénomène de remodelage inverse, son incidence sur le pronostic et rechercher des facteurs prédictifs cliniques de l'évolution. 31% de patients n'ont pas de signe de remodelage inverse, 20.6% sont super répondeurs. Les patients présentant un remodelage inverse sont moins symptomatiques. En analyse univariée : la cardiopathie ischémique, une durée plus longue d'évolution de la maladie et une FEVG plus altérée au moment de l'implantation du défibrillateur multisite sont des facteurs prédictifs d'une non réponse à la resynchronisation.
