La prise en charge d'un patient dément à domicile a un réel impact sur la qualité de vie des aidants. Dans ce contexte, et pour améliorer l'aide apportée aux aidants, il nous a semblé intéressant de nous pencher sur leur vécu et leurs attentes en termes d'aide et de soutien. Pour y parvenir, nous avons réalisé une étude qualitative auprès de 11 aidants familiaux de patients atteints de maladie d'Alzheimer pris en charge à domicile. Notre étude a montré que malgré le développement des services d'aide proposés aux aidants, les aidants utilisent peu ces services. Le répit des aidants, indispensable pour son équilibre psychique, est insuffisant. Le médecin traitant occupe une place centrale dans la prise en du malade et de son aidant. La qualité du suivi médical qu'il met en place avec l'aidant doit permettre de prévenir au maximum son épuisement. Le soutien apporté à l'aidant doit l'aider à mettre de côté ce sentiment de culpabilité qu'il éprouve si souvent, facteur limitant important dans la mise en place et l'acceptation des aides. Ce travail nous a permis de mieux comprendre le ressenti des aidants, préalable indispensable pour une prise en charge optimale du malade et de son aidant.

Pour que cette prise en charge soit la plus pertinente possible, il est important que les pharmaciens connaissent le parcours de soins de la maladie et les différents dispositifs d'aides (financières, juridiques, matérielles et humaines) auxquels les patients et leurs proches aidants peuvent recourir. Dans cet objectif, une revue bibliographique a été effectuée afin de décrire le parcours de soins de la maladie d'Alzheimer et maladies apparentées et l'ensemble des structures et des aides destinées à ces personnes. La partie expérimentale a permis l'élaboration de deux outils synthétiques et pratiques à destination des équipes officinales. Ces outils ont pour objectif de synthétiser les différentes étapes du parcours de soins et de mettre à disposition des informations sur les principaux dispositifs et structures vers lesquelles l'équipe officinale peut orienter le patient et son proche aidant. Ces outils ont été développés sous forme de posters à afficher dans les pharmacies d'officine, après validation d'un groupe de travail hospitalier et officinal. Ces derniers permettront donc aux équipes officinales d'avoir à disposition les informations nécessaires pour orienter et accompagner au mieux les patients atteints de la maladie d'Alzheimer et maladies apparentées et leurs proches aidants.
