Book excerpt: Les parodontites dites « agressives localisées » ont été spécifiquement associées à la présence de l'Aggregatibacter actinomycetemcomitans (A.a). Cependant, A.a est aussi détecté dans d'autres formes de parodontites mais de manière plus variable, posant la question de la pertinence du diagnostic bactérien du A.a en parodontologie, en particulier au regard de la prescription d'antibiothérapie. Objectif : l'objectif de cette étude est d'évaluer les associations entre l'infection au A.a, et les profils cliniques/microbiologiques observés chez des patients adultes. Matériels et Méthodes : les données démographiques, parodontales, et microbiologiques des patients ayant consulté de 2014 à 2018 en parodontologie au cabinet du Dr Pierre Simonis à Strasbourg ont été recueillies. L'analyse microbiologique quantitative par Real Time-PCR (test Carpegen®) des 6 principaux parodonto-pathogène, dont A.a, Treponema denticola (T.d), et Porphyromonas gingivalis (P.g). Deux groupes de patients ont été constitués en se basant sur la détection (A.a+) ou non (A.a-) de A.a et classés en fonction des classification parodontale AAP 1999 et AAP/EFP 2018. Discussion : le nombre de patients sélectionnés est de 88 A.a+ et 105 A.a-. Les patients A.a+ sont plus jeunes, ont moins de dents absentes, et une perte osseuse/âge plus marquée. Les autres paramètres démographiques et parodontaux, sont comparables entre les deux groupes. Le pourcentage de T.d est significativement augmenté dans le groupe A.a+. Les parodontites « agressives localisées » se retrouvent essentiellement dans le groupe A.a+ (4 patients sur 5). Les pourcentages de parodontites sévères « stade III/IV-grade B (63%) ou -C (45%) » sont similaires entre A.a+ et A.a-.
