Book excerpt: Le diagnostic de la maladie parodontale est réalisé à l'aide d'un examen clinique et radiologique. Aujourd'hui, c'est le status radiographique qui est défini comme examen de référence. En parodontologie, l'indication du CBCT est limitée à certains cas. Afin d'évaluer l'intérêt de réaliser un examen radiographique tridimensionnel systématique dans le cadre du bilan parodontal initial, nous avons réalisé une étude dans un cabinet libéral ayant un exercice exclusif en parodontologie. Nous avons, dans un premier temps, comparé le nombre de lésions infra-osseuses, endo-parodontales, furcatoires et endodontiques indentifiables sur des radiographies 2D et 3D. Dans un deuxième temps, les lésions infra-osseuses identifiées ont été caractérisées (profondeur, largeur mésio-distale et angle) sur les examens 2D et 3D. Cette étude a permis de montrer que le CBCT permet de détecter des lésions non visibles à l'examen 2D courant ; il permet, de plus, d'établir un diagnostic plus précis des lésions infra-osseuses, et met en évidence le nombre de parois osseuses, ce qui n'est pas possible en 2D.
