Book excerpt: La carie dentaire, maladie connue depuis l’époque mésopotamienne, reste au XXIème siècle le troisième fléau mondial. A partir du XIXème siècle, des scientifiques européens et américains ont recherché l’étiologie précise de la carie et les facteurs prédisposants à son apparition. Par la suite, au XXème siècle, les pays industrialisés ont pris conscience de ce qu’il convient d’appeler un problème de santé publique au vu des répercussions sur la santé générales et sur l’économie de la santé engendrées par la carie. Des stratégies de prévention bucco-dentaire se sont alors logiquement mises en place dans ces pays ciblant en général la population jeune des moins de 20 ans. La France, en particulier, depuis les années 1980, a instauré une politique de prévention bucco-dentaire nécessitant la participation active du gouvernement, des professionnels de santé, tout particulièrement des chirurgiens-dentistes, et des associations à but non lucratif. Parallèlement à la France, les Etats européens et les pays d’Outre-Atlantique (les Etats-Unis par exemple) ont développé des campagnes d’information et d’éducation bucco-dentaire. Leur stratégie de prévention n’apparaît pas semblable au modèle français en raison d’une organisation du système de santé ou de leur politique de santé publique différente. Les résultats obtenus en matière de risque carieux chez les jeunes de moins de 20 ans apparaissent divers, en conséquence, selon les pays observés. Quant aux pays en voie de développement (Afrique) ou nouvellement industrialisés (Amérique latine), la prévalence carieuse s’est accrue dans la population suite à la modernisation de son mode de vie. Quand est-il de la prévention bucco-dentaire dans ces Etats ?

Après une brève présentation de la problématique de la prévention bucco-dentaire en France, la démarche diagnostique en prévention carieuse est présentée puis les principales stratégies de lutte contre la survenue de lésions carieuses sont décrites. Ces stratégies sont notamment le contrôle de la plaque dentaire, l'augmentation de la résistance de l'émail de l'hôte, l'hygiène alimentaire. Les enjeux de la communication entre le patient et le praticien permettant l'apprentissage d'un mode de vie compatible avec la conservation de la dent fonctionnelle sont également développés.
