Dans la prise en charge des lésions carieuses, l'avancée dans la compréhension des phénomènes d'inflammation/régénération pulpaire, des cellules souches et de la biologie pulpaire a permis de mettre en avant des thérapeutiques moins invasives. Dans la gestion de la carie profonde, la préservation de la vitalité devra être le principal objectif. Pour cela, de nombreuses approches se sont développées. L'éviction en deux temps, ou « stepwise excavation technique », est une méthode d'éviction qui permet l'arrêt du processus carieux, l'arrêt de l'inflammation pulpaire, la reminéralisation de la dentine affectée et la dentinogenèse tertiaire. Lorsque la pulpe est exposée, suite à un traumatisme ou une effraction iatrogène, le pronostic quant à la conservation de sa vitalité est impacté. La technique d'éviction en deux temps permet de limiter le nombre d'effractions pulpaires lors de l'éviction carieuse. Par ailleurs, le taux de succès est relativement élevé, mais il est à mettre en regard avec ceux d'autres techniques comme l'éviction sélective par exemple.

The students will receive their language assignment after the selection phase where they will receive a language starter kit that allows them to begin language training while waiting to return to Fort Bragg for Phase II. The 3rd Bn, 1st SWTG (A) is responsible for all language training at the USAJFKSWCS. The Special Operations Language Training (SOLT) is primarily a performance-oriented language course. Students are trained in one of ten core languages with enduring regional application and must show proficiency in speaking, listening and reading. A student receives language training throughout the Pipeline. In Phase IV, students attend an 8 or 14 week language blitz depending upon the language they are slotted in. The general purpose of the course is to provide each student with the ability to communicate in a foreign language. For successful completion of the course, the student must achieve at least a 1/1/1 or higher on the Defense Language Proficiency Test in two of the three graded areas; speaking, listening and reading. Sans traitement la lésion progresse et l'atteinte pulpaire devient irréversible. La prise en charge de la carie profonde est donc importante et doit permettre la préservation de la vitalité pulpaire dès que possible. Ainsi, plusieurs protocoles d'éviction de la carie profonde ont été étudiés ; l'éviction totale, la méthode Step Wise et, enfin, l'éviction partielle.Cette thèse reprend l'ensemble des différentes méthodes, et met en lumière le traitement par éviction partielle : son approche, les connaissances actuelles et les recherches en cours. Une éviction de ce type permet au praticien de diminuer le risque d'effraction pulpaire en collant directement sur de la dentine affectée (laissée en fond de cavité). Le pronostic vital de la dent est ainsi amélioré. Au final, nous comprenons au travers de cette thèse qu'il est préférable de privilégier l'éviction carieuse partielle sous contrôle visuel et champ opératoire. Aujourd'hui, un protocole de collage peut être placé en contact de la dentine affectée. Qu'en sera-t-il demain ?

En effet, les principes d'économie tissulaire et de préservation de la vitalité pulpaire doivent être au centre des priorités du chirurgien-dentiste afin d'augmenter la durée de vie des dents sur l'arcade de nos patients. Dans le cadre de caries profondes sur les dents postérieures permanentes matures, la pratique des praticiens est généralement en faveur d'une élimination complète de la lésion carieuse, aboutissant souvent à l'exposition pulpaire et à la perte de la vitalité dentaire, malgré une absence de symptomatologie pulpaire. Cette situation clinique de carie profonde juxta pulpaire sans symptomatologie sur dents permanentes postérieures matures est régulièrement rencontrée. C'est pourquoi il est important d'approfondir ses connaissances actuelles afin d'avoir une pratique préservant au maximum l'organe dentaire. Après quelques rappels, les différentes techniques d'excavation carieuse possibles seront exposées dans la première partie de cette thèse. Puis il sera démontré qu'il n'est pas nécessaire de retirer toute la dentine cariée, dans le cadre de caries profondes, pour obtenir un arrêt de l'évolution de la pathologie carieuse. Ensuite, la méthode de reconstitution en technique directe sera développée afin de conserver de manière durable l'organe dentaire. Enfin, une étude multicentrique prospective menée en France dans 13 hôpitaux universitaires et 2 cabinets privés, l'étude DEep Caries Treatment (DECAT) sera présentée. La dernière partie sera consacrée à un cas clinique réalisé par l'auteur dans un cabinet d'omnipratique qui permettra d'appréhender de manière simple et concrète la mise en place thérapeutique la plus adaptée.
