Book excerpt: Face à une lésion carieuse profonde, le traitement de choix a longtemps été l'éviction complète du tissu carieux, impliquant un risque d'effraction pulpaire pouvant compromettre la conservation de la vitalité pulpaire. Une meilleure approche biologique du processus carieux et de la cicatrisation pulpo-dentinaire, associée au développement des matériaux biocompatibles, ont permis de reconsidérer cette approche traditionnelle au profit de plusieurs thérapeutiques minimalement invasives : l'éviction carieuse en deux temps ou la « stepwise excavation » et la technique en un temps ou l'éviction partielle. L'éviction en un temps, en s'exemptant du risque d'effraction pulpaire lors de la ré-intervention, décrite dans la technique en deux étapes, semble préférable. Cependant de nouveaux essais cliniques sont nécessaires quant à la prise de conscience de l'efficacité de ces techniques conservatrices par les chirurgiens-dentistes.

Sans traitement la lésion progresse et l'atteinte pulpaire devient irréversible. La prise en charge de la carie profonde est donc importante et doit permettre la préservation de la vitalité pulpaire dès que possible. Ainsi, plusieurs protocoles d'éviction de la carie profonde ont été étudiés ; l'éviction totale, la méthode Step Wise et, enfin, l'éviction partielle.Cette thèse reprend l'ensemble des différentes méthodes, et met en lumière le traitement par éviction partielle : son approche, les connaissances actuelles et les recherches en cours. Une éviction de ce type permet au praticien de diminuer le risque d'effraction pulpaire en collant directement sur de la dentine affectée (laissée en fond de cavité). Le pronostic vital de la dent est ainsi amélioré. Au final, nous comprenons au travers de cette thèse qu'il est préférable de privilégier l'éviction carieuse partielle sous contrôle visuel et champ opératoire. Aujourd'hui, un protocole de collage peut être placé en contact de la dentine affectée. Qu'en sera-t-il demain ?

Dans la gestion de la carie profonde, la préservation de la vitalité devra être le principal objectif. Pour cela, de nombreuses approches se sont développées. L'éviction en deux temps, ou « stepwise excavation technique », est une méthode d'éviction qui permet l'arrêt du processus carieux, l'arrêt de l'inflammation pulpaire, la reminéralisation de la dentine affectée et la dentinogenèse tertiaire. Lorsque la pulpe est exposée, suite à un traumatisme ou une effraction iatrogène, le pronostic quant à la conservation de sa vitalité est impacté. La technique d'éviction en deux temps permet de limiter le nombre d'effractions pulpaires lors de l'éviction carieuse. Par ailleurs, le taux de succès est relativement élevé, mais il est à mettre en regard avec ceux d'autres techniques comme l'éviction sélective par exemple.
