Faisant partie d'une profession médicale à compétences limitées, la sage-femme possède un droit de prescription concernant en particulier les morphiniques, dont la dernière modification date du 23 février 2004. Un questionnaire envoyé dans les 22 maternités de Lorraine, aux sages-femmes exerçant en salle de naissances dans les trois niveaux de soins, a permis de faire la synthèse sur l'utilisation actuelle des morphiniques par les sages-femmes de salle de naissances. Les morphiniques utilisés en début de travail sont minoritaires par rapport à ceux utilisés sous délégation, notamment dans le cas de l'anesthésie péridurale et dans une moindre mesure, dans l'utilisation de protocole Nubain® ou de l'anesthésie en mode PCEA (analgésie contrôlée par le patient). Les sages-femmes utilisent peu leur droit de prescription propre des morphiniques mais ceux-ci mériteraient d'être développés, spécialement dans le cadre de la prise en charge du faux début de travail, de la mort fœtale in utero ou de l'interruption volontaire médicale de grossesse.

Book excerpt: Objectif de l'étude : cette étude proposait d'évaluer la satisfaction des patientes du CHRUB à propos de l'utilisation de la morphine en salle de naissance, puis de décrire les modalités d'utilisation de ce produit et ses conséquences. Matériel et méthodes : étude descriptive prospective par questionnaire, réalisée du 10 juin et au 10 septembre 2016, auprès de 83 patientes ayant reçu de la morphine en intramusculaire, menée au CHRUB. Résultats : l'étude a montré que 85.2% des patientes se disaient satisfaites de cette thérapeutique et parmi elles, 68.4% ont déclaré avoir ressenti un soulagement. L'EVN a diminué en moyenne de 2.6 points. Par ailleurs, plus de la moitié des patientes ont déclaré avoir ressenti des nausées, vomissements, vertiges et somnolence. Conclusion : soulager et accompagner les patientes face à la douleur fait partie intégrante de la prise en charge des sages-femmes. L'injection de morphine en pré-travail permet d'obtenir une diminution de la douleur. Cependant, cette pratique provoque des effets indésirables importants et nécessite une surveillance soutenue. Une comparaison des effets de la morphine pourrait être envisagée avec d'autres méthodes thérapeutiques comme la nalbuphine.

Ceci amène à la question de départ : quels critères conditionnent l'utilisation de la nalbuphine et du protoxyde d'azote par la sage-femme en salle de naissances?
