Sage-femme, acteur de première ligne joue un rôle de repérage essentiel durant cette période de vulnérabilité psychique. Matériel & Méthode - Les sages-femmes libérales et hospitalières de Haute-Normandie (Eure et Seine Maritime) ont été interrogées par questionnaire. Résultats - 154 sages-femmes ont constitué l'échantillon de notre étude. Nous obtenons une note de 2,47/6 : les sages-femmes ont un faible niveau de connaissance. 27% n'ont jamais entendu parler de l'ESPT et de ses spécificités en périnatalité. 25% des sages-femmes ont déjà pris en charge un ESPT et mis en place des mesures particulières. 97% des sages-femmes considèrent avoir une formation insuffisante. 84% des sages-femmes pensent que de nouveaux outils pourraient être mis en place. Discussion - Deux sages-femmes sur trois ont un faible niveau de connaissance et ne se sentent pas capables de prendre en charge l'ESPT. Elles semblent confondre l'ESPT et le vécu traumatique des évènements. Elles sont en demande d'outils et jugent leur formation insuffisante. Il est donc important de renforcer la formation (initiale et continue) et de leur apporter des outils de repérage pertinents (échelles, plaquette, table ronde, cas clinique). Conclusion - L'ESPT en périnatalité est un sujet encore méconnu des sages-femmes et nécessite de mettre en place des moyens d'action pour son repérage.

Book excerpt: Contexte: Après l’annonce d’un décès périnatal qui, dans le monde de la maternité, fait choc peuvent se développer des troubles psychotraumatiques et notamment l’état de stress post traumatique, qui par sa fréquence, ses conséquences à court et long terme, notamment lors d’une grossesse suivante, ainsi que son association à un trouble dépressif comorbide en font une question qui nécessite d’être connue pour être pris en compte. Méthode: A partir d’une revue de littérature, nous avons tenté de définir le caractère traumatique du deuil périnatal, d’identifier les facteurs qui faisaient de ce deuil un traumatisme afin d’en améliorer le dépistage et le diagnostic pour en permettre une prise en charge adaptée. Résultats: En France le taux de mortalité périnatale s’élève à 9,3 pour mille naissances. L’accouchement et la naissance d’un enfant mort-né, peuvent être considérés comme événement traumatique, facteur déclenchant d’apparition du trouble psychotraumatique. Les études menées auprès de femmes ayant subi une perte périnatale, évaluent la prévalence de l’état de stress post traumatique à 25% à un mois et à 7% quatre mois après le décès. Au troisième trimestre de la grossesse suivante, 21% des femmes remplissent les critères cliniques d’un ESPT. Les facteurs de risque les plus reconnus sont: le décès à un âge gestationnel tardif, le travail long et douloureux, le soutien familial inadéquat, les manoeuvres obstétricales, la survenue d’une grossesse suivante moins d’un an après, avoir vu et touché son enfant mort et la présence d’un épisode de dissociation péritraumatique. La comorbidité dépressive est présente pour 36%des femmes souffrant d’ESPT. Lors de la naissance de l’enfant suivant, il est parfois constaté des difficultés dans le processus d’attachement à l’enfant, celui-ci lui rappelant l’événement traumatique, provoquant chez la mère un rejet, une grande froideur, jusqu'à le tenir responsable voire lui nuire. Conclusion: Ce travail a montré qu’il était nécessaire de sensibiliser les équipes obstétricales et psychiatriques à l’existence de ces manifestations psychotraumatiques, au repérage des facteurs de risque, à l’importance d’assurer le soutien le plus adapté à la situation, afin de mettre en route le processus de réappropriation psychique de cette naissance.

Dans un premier temps, nous présentons un état de l’art sur le « stress post terrorisme » chez le jeune enfant. La littérature scientifique est limitée en particulier chez l’enfant avant l’entrée dans le langage. La symptomatologie non spécifique échappe aux classifications diagnostiques inspirées de l’adulte et son repérage est limité par le manque de diffusion des outils diagnostiques spécifiques de cette tranche d’âge. Les études antérieures montrent que l’enfant est rendu vulnérable par son environnement et que sa dépendance majeure à l’état psychique parental est déterminante dans le processus de résilience. Les grandes lignes de la prise en charge thérapeutique sont discutées mais ne sont pas établies à ce jour. Dans un second temps, nous soumettons les résultats d’une étude clinique observationnelle descriptive transversale réalisée auprès de mères ayant accouché au moment de l’attentat du 13 novembre 2015 à Paris. Les conclusions de l’étude confirment la présence de symptômes de stress post terrorisme en périnatalité à l’impact traumatique durable pour la mère, l’enfant et le lien mère-bébé, modulé par des facteurs de risques individuels et collectifs. Dans un troisième temps, nous exposons trois études de cas d’enfants de moins de 3 ans exposés aux attentats de Nice et de Paris qui mettent en évidence la complexité de l’expression traumatique du bébé et du jeune enfant. En effet, inscrits dans une dynamique développementale singulière, leurs symptômes échappent aux parents pris dans leur propre détresse post traumatique et relèvent d’une approche familiale globale. Connaissant les facteurs de vulnérabilité prédictifs de l’apparition de troubles post traumatiques, une évaluation ciblée des femmes et jeunes enfants exposés pourrait s’instaurer de manière systématique en cas de trauma potentiel.

Book excerpt: Child and adolescent psychiatry hosts a range of diverse epistemological positions regarding the origin of psychical suffering, from fully endogenous (e.g. genetic) to mostly exogenous (e.g. family trauma, etc.). The complexity of clinical situations generally precludes such epistemologies to require exclusive therapeutic strategies: psychodynamic psychotherapy can be fruitful in the context of monogenic genetic illnesses (at the family or individual level), while pharmacology can be a necessary tool in a variety of difficult relational contexts or personality issues (e.g. in adolescence). Thus nowadays, the most promising therapeutic perspectives in child and adolescent psychiatry attempt to do justice to the polyfactorial complexity of mental suffering (notably by refining their psychopathologies), by drawing on e.g. biopsychosocial or epigenetic models – even more so as prevention policies ask for longitudinal studies to help with the early detection of potential future troubles.
