La mise en oeuvre d'interventions efficaces permettant d'atténuer ce phénomène s'avère une condition sine qua non dans les services de soins en santé mentale. Objectifs 1. Déterminer l'efficacité des interventions utilisées pour diminuer la violence des personnes souffrant de troubles mentaux 2. Évaluer l'utilisation d'un outil d'évaluation du risque de violence par les professionnels de santé dans une unité de santé mentale. Méthodes Deux études séquentielles basées sur le modèle de passage des connaissances à la pratique ont été réalisées : 1. La première étude consistait en une revue systématique de la littérature basée sur la méthode développée par la Collaboration Cochrane pour les revues systématiques d'interventions et sur les lignes directrices PRISMA. 2. Une étude descriptive transversale a été effectué dans l'unité interne de santé mentale d'un hôpital de l'est de l'Ontario. Résultats 1. Parmi les 5 039 références identifiées, 7 articles ont répondu aux critères de sélection. Au total, cinq interventions visant la diminution des comportements agressifs chez les personnes ayant des troubles mentaux ont été mises en lumière. L'évaluation du risque de violence de façon quotidienne a été définie comme étant l'intervention la plus efficace. 2. Parmi les 45 personnes éligibles à cette étude, 26 d'entre eux (57,8 %) ont répondu au questionnaire : 92,3 % des répondants ont déclaré avoir été exposés à un acte de violence et 62 % ont affirmé qu'ils avaient déjà utilisé l'outil d'évaluation du risque de violence disponible dans leur unité, mais pas de façon quotidienne. Des obstacles liés au manque de connaissances de l'outil, au manque de ressources et de temps, ainsi qu'aux attitudes négatives envers les patients et à l'égard de l'outil ont été décelés. Conclusion Bien que les preuves appuient l'utilisation quotidienne de l'évaluation du risque de violence comme moyen pour prévenir et diminuer les comportements agressifs, cette intervention ne constitue pas une pratique courante dans l'unité de santé mentale. Des obstacles cognitifs, organisationnels et comportementaux ont ainsi été soulevés.

Although a wide range of evidence-based psychosocial interventions are currently in use, most consumers of mental health care find it difficult to know whether they are receiving high-quality care. Although the current evidence base for the effects of psychosocial interventions is sizable, subsequent steps in the process of bringing a psychosocial intervention into routine clinical care are less well defined. Psychosocial Interventions for Mental and Substance Use Disorders details the reasons for the gap between what is known to be effective and current practice and offers recommendations for how best to address this gap by applying a framework that can be used to establish standards for psychosocial interventions. The framework described in Psychosocial Interventions for Mental and Substance Use Disorders can be used to chart a path toward the ultimate goal of improving the outcomes. The framework highlights the need to (1) support research to strengthen the evidence base on the efficacy and effectiveness of psychosocial interventions; (2) based on this evidence, identify the key elements that drive an intervention's effect; (3) conduct systematic reviews to inform clinical guidelines that incorporate these key elements; (4) using the findings of these systematic reviews, develop quality measures - measures of the structure, process, and outcomes of interventions; and (5) establish methods for successfully implementing and sustaining these interventions in regular practice including the training of providers of these interventions. The recommendations offered in this report are intended to assist policy makers, health care organizations, and payers that are organizing and overseeing the provision of care for mental health and substance use disorders while navigating a new health care landscape. The recommendations also target providers, professional societies, funding agencies, consumers, and researchers, all of whom have a stake in ensuring that evidence-based, high-quality care is provided to individuals receiving mental health and substance use services.

Book excerpt: Il est désormais bien documenté qu'un trouble mental, comme les troubles psychotiques, a des impacts importants sur la famille de la personne atteinte. Lors d'un premier épisode psychotique (PEP), ce sont plus particulièrement les parents qui sont sollicités pour exercer un rôle de soutien pour leur jeune adulte et contribuer aux interventions précoces favorisant la réadaptation et le rétablissement. Les programmes d'intervention précoce basés sur les bonnes pratiques de traitement de la schizophrénie incluent également de l'intervention auprès des familles. Si des interventions familiales tendent à être offertes, on en connait peu sur les facteurs qui prédisent l'adaptation des parents lorsqu'ils exercent un rôle de soutien dans ce contexte particulier. Dans le cadre de la présente recherche, un modèle d'adaptation a été testé sur un échantillon de 58 parents et beaux-parents qui ont reçu des services d'une clinique spécialisée dans le traitement des troubles psychotiques en début d'évolution. La posture initiale qui a guidé le choix des variables était que de s'adapter dans le contexte d'un PEP peut être une occasion de développement, voire même de croissance personnelle. Les résultats obtenus aux analyses statistiques permettent de comprendre l'adaptation de manière systémique en tenant compte de l'interinfluence des facteurs personnels (certaines caractéristiques des parents) et des facteurs environnementaux (caractéristiques liées à l'intervention). Le modèle d'adaptation final invite à dégager des cibles d'interventions qui peuvent entraîner le développement d'un pouvoir d'agir plus grand chez les parents, de même que le maintien de leur fonctionnement social. La participation à des stratégies actives d'adaptation comme celle de s'engager dans l'intervention familiale et de développer une relation de collaboration avec les travailleuses sociales sont des contributions nouvelles au phénomène de l'adaptation chez des parents. Miser sur l'intervention précoce, favoriser la participation active des parents et des personnes atteintes d'un trouble psychotique à des rôles sociaux valorisants et encourager le recours à une gamme de services offerts en partenariat font partie des recommandations formulées à la lumière des résultats obtenus dans cette thèse.
