Aujourd'hui le cahier des charges et les enjeux de l'implantologie moderne vont bien au-delà de l'ostéo-intégration et de l'intégration fonctionnelle. La demande est majoritairement esthétique, tant au niveau du « blanc que du rose » / « pink and white esthetic ». Les deux grands challenges pour assurer la pérennité de nos travaux sont : la gestion de l'espace biologique et de l'environnement péri-implantaire. Les résultats à long terme seront influencés par la conservation du volume osseux des secteurs antérieurs. La Socket-Shield technique, ou technique d'implantation immédiate avec maintien partiel d'une racine dentaire est une technique développée par Hürzeler et al. et qui a pour vocation le maintien de la corticale vestibulaire de l'alvéole dentaire après extraction. Cette dernière est constituée de « bundle bone » ; os très labile qui ne doit sa survie et son maintien qu'à l'existence d'un ligament alvéolo-dentaire. C'est donc ce « bouclier radiculaire » avec le ligament parodontal qui vont nous garantir la réussite du traitement. Les objectifs de cette thèse sont, dans un premier temps, d'étudier les phénomènes de remaniement osseux après extraction dentaire ; dans un second temps, de comprendre les critères d'évaluation du traitement implantaire en zone esthétique. Nous comparerons ensuite l'évolution des différentes techniques qui ont permis d'aboutir à la « Socket-Shield ». Enfin, nous étudierons à travers différents cas cliniques les avantages et inconvénients de cette technique, ainsi que le recul et les complications associés à cette dernière.

Book excerpt: L'extraction implantation immédiate s'est développée dans le but de réduire le délai de mise en place de l'implant après extraction dentaire. Cette technique permet ainsi de s'affranchir de la cicatrisation osseuse post-extractionnelle de l'alvéole. Une analyse de la littérature est réalisée afin d'exposer l'ensemble des avantages et inconvénients de ce procédé, ainsi que les consensus cliniques, en les comparant aux autres techniques d'implantations. La survie implantaire lors de l'extraction implantation immédiate dépasse 90%, quelle que soit la situation clinique et à condition d'obtenir la stabilité primaire de l'implant lors de sa mise en place. La présence d'une infection péri-apicale ne diminue pas ce taux de survie implantaire si le protocole de mise en place des implants au niveau d'un site infecté est respecté. “La pose d'implant, immédiatement après l'extraction dentaire, réduit de façon significative le remodelage osseux post-extractionnel”. Cette affirmation est aujourd'hui remise en question. En effet, la mise en place d'implants, qu'elle soit immédiate ou différée, n'influence pas le remodelage osseux. Les modifications osseuses influencent directement celles des tissus mous sus-jacents. Il est montré que les récessions gingivales sont plus marquées à la mandibule et en présence d'un biotype gingival fin.

Une extraction-implantation immédiate est alors réalisée. Hors, fréquemment, la dent devant être extraite est infectée. Devant les avantages non négligeables de gain de temps et de confort augmenté pour le patient, la mise en place d'un implant dans un site infecté (parodontalement ou endodontiquement) a longtemps été considérée comme une contre-indication. Qu'en est-il aujourd'hui ? Nous aborderons dans une première partie la notion de site infecté en implantologie. Il a longtemps été considéré que l'implant jouait le rôle de tuteur osseux évitant ainsi la résorption osseuse post-extractionnelle. En effet l'intérêt premier de ce protocole était de raccourcir le temps de traitement implantaire, de préserver l'os alvéolaire après extraction et de gérer les tissus mous. Cependant de récentes études tendent à prouver que l'implant n'empêche pas cette résorption. Celles-ci remettent donc en question le choix de cette thérapeutique dans l'arbre décisionnel. L'objet de ce travail est de définir quelles sont les indications exactes permettant la mise en place d'implants post-extractionnels.
