Book excerpt: La grossesse à l'adolescence est un phénomène social et de santé publique qui demeure une préoccupation à la fois dans les pays riches et dans les pays à revenu faible ou intermédiaire. Chaque année, des millions d'adolescentes tombent enceintes dans le monde et la plupart de ces grossesses se terminent par une naissance. Quatre-vingt-quinze pour cent de ces grossesses surviennent dans des pays à revenu faible ou intermédiaire. Ce type de grossesse peut entraîner diverses conséquences négatives pour l'adolescente, sa famille et la société dans son ensemble. À l'instar de tous les pays à revenu faible, Haïti a un taux de fécondité à l'adolescence assez élevé. Cependant, un manque important subsiste en matière de travaux ayant analysé et documenté le vécu expérientiel de la grossesse et de la maternité à l'adolescence dans le contexte haïtien. En vue de combler la lacune existante entourant ce phénomène, cette thèse vise à : décrire les dimensions d'influence associées au sexe et au genre, telles que perçues par les adolescentes enceintes et autres acteurs qui engendrent la survenue de la grossesse et la maternité à l'adolescence dans le contexte haïtien ; identifier les différentes perceptions de la grossesse et de la maternité chez les adolescentes haïtiennes ; décrire les forces et les ressources mobilisées ainsi que les contraintes rencontrées par les adolescentes enceintes et leurs proches au cours de l'expérience de la transition à la maternité ; et identifier les recommandations de pratiques communautaires et cliniques qui émergent du discours des participants favorisant la mise en œuvre de stratégies pouvant contribuer à la prévention et à l'amélioration du vécu expérientiel de la grossesse chez les adolescentes dans le contexte haïtien. S'inspirant du pragmatisme du courant du constructivisme radical, un devis de recherche qualitative développé à partir de l'enquête sociale de Dewey a été utilisé dans le cadre de cette thèse. L'étude a été menée dans les départements du Nord et du Nord-Est d'Haïti et les données ont été collectées à l'aide d'entrevues individuelles semi-structurées. Pour construire le guide d'entretien et pour structurer l'analyse des données, deux perspectives théoriques ont été utilisées pour conduire ces étapes de l'étude, à savoir : l'intersectionnalité et la théorie de la justice sociale de Nancy Fraser. Au total, 69 participants ont été interviewés, dont 33 adolescentes enceintes, 17 proches des adolescentes enceintes, 15 professionnels de la santé et quatre responsables d'organismes travaillant avec les adolescentes. Après transcription, vérification et validation, les 69 verbatims en format Word ont été exportés vers le logiciel QDA Miner version 6.0.5 pour l'analyse thématique. Pour analyser les données recueillies, l'étudiant chercheur a utilisé l'analyse thématique en continu décrite par Paillé et Mucchielli. Les résultats ont permis d'observer que les dimensions d'influence de la grossesse à l'adolescence sont liées à des identités sociales multiples, mais également à des inégalités structurelles et systémiques. En outre, la perception de la grossesse et de la maternité à l'adolescence représente un phénomène paradoxal et ambivalent, l'expérience pouvant être négative ou positive en fonction de l'adolescente, de son contexte et des structures et des systèmes sociopolitiques et religieux en place. Les adolescentes enceintes et leurs proches ont connu des difficultés économiques, sociales et psychologiques pendant la période de transition à la maternité. Pour prévenir la grossesse à l'adolescence et améliorer le vécu expérientiel des mères adolescentes, les résultats indiquent que des stratégies comme l'intervention multisectorielle, la protection des droits sexuels et reproductifs des adolescentes, la sensibilisation de masse, l'implication des adolescentes dans la mise en œuvre des programmes et l'amélioration des conditions de vie de la population sont à prioriser. Les résultats ont indiqué que la plupart des dimensions d'influence de la grossesse et de la maternité restent parfois hors du contrôle des adolescentes, car ces dernières sont confrontées à un déni de reconnaissance culturelle, de redistribution économique et de participation sociale. De ce fait, la grossesse et la maternité à l'adolescence ne forment pas une expérience homogène, puisqu'il existe une histoire et un parcours de vie derrière chaque situation. Cette étude a montré que la prévention de la grossesse à l'adolescence ne doit pas miser uniquement sur la promotion de l'utilisation de la contraception et l'éducation sexuelle qui visent principalement les corps biologiques des adolescentes. Les résultats ont suggéré également d'autres stratégies holistiques et communautaires pour y parvenir.
