Book excerpt: La prise en charge de malades atteints de pathologies chroniques est souvent associée à une mauvaise observance entraînant une altération de la qualité de vie de ces patients ainsi qu'une augmentation des dépenses de santé. L'éducation thérapeutique est une réponse à cette problématique. En effet, elle va au-delà de l'information et du simple conseil en visant l'acquisition de compétences. Les autorités encadrent cette discipline au sein de nombreux textes législatifs. Suite à la loi HPST, plusieurs décrets et arrêtés sont parus en août 2010, fixant les règles précises de la mise en place de programmes d'éducation thérapeutique. Ces différents points ont fait l'objet de la première partie de ce travail. Dans un deuxième temps, nous avons envisagé les quatre étapes successives structurant un programme d'ETP selon l'HAS : le diagnostic éducatif, l'établissement d'un contrat éducatif, les séances d'éducation à proprement parler et l'évaluation. L'utilisation de méthodes pédagogiques et de communication efficaces est nécessaire à cette pratique. Mais, est-il envisageable d'adapter la pratique de l'éducation thérapeutique du patient à l'officine ? Après une première expérience dans le service de Néphrologie à l'Hôpital d'adulte du CHU de Nancy, j'ai eu envie d'essayer de répondre à cette question en poursuivant cette aventure durant mon stage de pratique professionnelle. En partant des recommandations et de mon expérience en Néphrologie, j'ai établi un programme d'éducation dans mon officine de stage. Pour cela, j'ai créé des supports spécifiques pour chaque étape, du diagnostic à l'évaluation et pour chacune des trois pathologies sélectionnées (l'hypertension artérielle, l'insuffisance cardiaque et l'asthme). Douze patients ont été recrutés pour mon étude. A l'issue des séances, j'ai pu dresser un bilan concernant l'efficacité de l'éducation thérapeutique à l'officine. Il s'avère qu'il existe un bénéfice non négligeable en terme d'apprentissage pour les patients. Les connaissances concernant la maladie et ses traitements ont, en effet, progressé pour tous les patients. Quant au pharmacien, cette activité est très enrichissante et valorisante mais elle nécessite un investissement en temps et en moyens important. D'après le décret d'août 2010 relatif aux conditions d'autorisation des programmes d'ETP, un officinal ne peut envisager seul de développer un programme reconnu par les autorités de santé. Le pharmacien doit donc trouver un autre moyen de s'investir dans l'éducation thérapeutique notamment par le biais de réseaux ou d'associations. Le pharmacien, grâce à ses connaissances et ses compétences, a toute sa place dans l'élaboration de programmes d'éducation thérapeutique. Il ne tiendra qu'a lui d'y consacrer du temps, tout d'abord pour se former, puis pour l'exercer.
