Book excerpt: Introduction L'extraction dentaire est un geste potentiellement douloureux réalisé dans certains cas sous anesthésie générale. Les techniques d'anesthésie locorégionale par voie percutanée semblent procurer un bénéfice post opératoire non négligeable en complément de l'anesthésie générale. Type d'étude Etude prospective, randomisée, en double aveugle. Matériel et méthode Tous les patients sont opérés sous anesthésie générale, et hospitalisés en service d'ambulatoire. Un groupe de patient avec une ALR complémentaire (bloc maxillaire et/ou bloc mandibulaire), un groupe contrôle. Evaluation des paramètres cliniques (hémodynamique, douleur, NVPO, satisfaction, sommeil) et pharmacologiques (consommation de morphiniques, d'antalgiques, antiémétiques) per opératoire, post opératoire et au lendemain de l'intervention lors d'un appel téléphonique. Résultats 50 patients inclus, 25 par groupe. Pas de différences démographiques entre les groupes. Pour le groupe sans ALR, réinjections de sufentanyl chez 9 patients (36%), variation de fréquence cardiaque +7 bpm, variation de tension artérielle +lOmmhg, recours au tramadol chez 16 patients (64%), EVA1=3, EVA2=3, ENSJ1=3, ENSMI=3, pas de réveil chez 36% des patients. Pour le groupe ALR, réinjection de sufentanyl chez 2 patients (8%), variation de fréquence cardiaque +2 bpm, variation de tension artérielle +7mmhg recours au tram~dol chez 10 patient (40%), EV AI =0, EV A2=O, ENSJ1 =0, ENSM1= 1, pas de réveil chez 72% des patients. Discussion La réalisation des anesthésies locorégionale par voie percutanée en complément de l'anesthésie générale permet une meilleure stabilité hémodynamique per opératoire, une diminution de la consommation de sufentanyl per opératoire, une diminution de la douleur postopératoire avec un recours moindre aux antalgiques de niveau 2. Elle permet un meilleur sommeil et ses effets antalgiques se prolongent au moins 24 heures. Par ailleurs ces techniques d'anesthésies locorégionales par voie per cutanée semblent plus sures que celles par voie endobuccale, avec peu de complications. Quand est-il des bénéfices à plus long terme? Seule une nouvelle étude permettrait d'y répondre.

De plus, le représentant légal de l'enfant doit être informé de façon précise sur le déroulement et les conséquences de l'intervention. Les données actuelles de la littérature prennent en compte le profil postopératoire du patient et non le profil initial. En effet, nous pouvons constater à travers les différentes études que les patients traités sous anesthésie générale n'ont pas eu d'examen clinique préopératoire détaillé afin de comparer la situation avant et après l'intervention. Le but de ce protocole d'étude est d'évaluer les répercussions (croissance staturo-pondérale, fonctions physiologiques, psychologie) des soins et extractions dentaires sous anesthésie générale chez l'enfant en tenant compte de la situation initiale du patient. Autrement dit, le protocole d'étude va permettre d'évaluer l'impact de l'anesthésie générale sur l'enfant : des outils objectifs seront utilisés pour déterminer les répercussions.

In order to avoid misunderstanding these abbreviations, it is wise to refer to a reliable dic tionary, such as this one prepared by Heister. The abbreviation ED means, for instance, effective dose to the pharmacologist. However, it might also stand for emetic dose. Radiologists use the same abbreviation for erythema dose, and ED could also mean ethyl dichlorarsine. A com mon meaning of ECU is European currency unit, a meaning that might not be very often in scientific medical publications. ECU, however, also means environmental control unit or European Chiropractic Union. Hopefully, those making inventions and discoveries will make use of Heister's dictionary before creating new abbreviations when preparing manuscripts for scientific publications. It is a very worthwhile goal not to use the same abbreviation for several different terms, especially if it is already widely accepted to mean only one of them. It may be impossible, however, to achieve this goal in different scientific disciplines. Therefore, although it is wise for the abbreviations used in a publication to be defined, it is also very helpful for readers and writers to use a dictionary such as this one. The author deserves our warmest thanks since we know that compiling such a comprehensive dictionary is based upon incredibly hard effort.
