Book excerpt: Contexte : Le médecin généraliste tient une place importante dans le choix de la contraception, que ce soit en France ou à l'Île Maurice. Cependant, une méconnaissance sur la contraception existe chez les jeunes filles à l'Île Maurice avec de nombreuses grossesses non désirées et le recours à des IVG cependant interdites par la loi. En France, les femmes se disent bien informées sur ce sujet mais le taux de recours à l'IVG chez les jeunes filles reste néanmoins stagnant. Objectif : Faire une comparaison de l'abord de la première contraception, au niveau de l'information, de la prescription et de la prévention, entre deux pays très différents, la France et l'Île Maurice, afin d'identifier les forces et les freins de cette consultation. Méthode : Une étude qualitative par entretiens individuels semi-dirigés auprès de 6 médecins généralistes du Nord-Pas-de-Calais et de 6 médecins généralistes de l'Île Maurice, avec retranscription intégrale avant triangulation. Une analyse par théorisation ancrée a été réalisée jusqu'à suffisance des données. Résultats : En France et à l'Île Maurice, la majorité des médecins généralistes prescrit une contraception en tenant compte des champs médicaux et non médicaux de la patiente. Cependant, la prise en charge des mineures reste floue, et la sexualité, nommée comme thème primordial à évoquer, n'est que peu abordée dans les deux pays. Le rôle de prévention est quasi inexistant à l'Île Maurice. Conclusion : Les résultats de cette étude tendent vers la délivrance d'une information complète et vers un respect des souhaits de la patiente. En France, un rappel de l'anonymisation des consultations des mineures ainsi qu'une sensibilisation à la sexualité via des formations seraient judicieux. A l'Île Maurice, des recommandations uniques ainsi qu'un travail sur l'abord de la sexualité pourraient être bénéfiques dans la prise en charge des patientes et de leur contraception.

La fécondité d'une femme correspond à presque 40 ans de sa vie. Le médecin généraliste est un acteur situé en première ligne dans la prescription des moyens de contraception. Une consultation globale structurée et actualisée est nécessaire pour une prise en charge optimale. Objectif : Évaluer les pratiques professionnelles lors de cette primo-consultation contraception en cabinet de médecine générale en Martinique et recherche de l'impact de leur formation initiale et secondaire en gynécologie. Méthode : Étude descriptive observationnelle transversale par questionnaire en ligne auprès des 288 médecins généralistes libéraux de la Martinique de février à mai 2017. Le questionnaire était basé sur les recommandations de la HAS. Résultats : 74 médecins ont répondu, soit un taux de réponse de 25%. Le modèle BERCER semble incontournable pour cette consultation. Il est peu connu par les médecins martiniquais tout comme les recommandations de la HAS, mais sont globalement appliqués. La démarche éducative est néanmoins peu réalisée, par manque de temps et de formation. Les médecins auraient préféré une formation plus approfondie et plus orientée sur la pratique permettant ainsi une proposition optimale du contraceptif et une meilleure information sur la conduite à tenir en cas d'échec (pilules, préservatifs ou méthodes naturelles). Conclusion : La sexualité, au sens large, est un sujet tabou difficilement abordable auprès des patientes. Une formation personnelle complémentaire est aujourd'hui nécessaire pour mieux maîtriser cette première consultation de contraception. Depuis quinze ans, de multiples études réalisées concluant à une carence de la formation théorique et pratique se succèdent. Il semble impératif d'ajouter au cursus médical, un stage en consultations de contraception et de proposer aux praticiens des outils simples pour acquérir les réflexes à la pratique au cabinet.

Book excerpt: Mitochondrial replacement techniques (MRTs) are designed to prevent the transmission of mitochondrial DNA (mtDNA) diseases from mother to child. While MRTs, if effective, could satisfy a desire of women seeking to have a genetically related child without the risk of passing on mtDNA disease, the technique raises significant ethical and social issues. It would create offspring who have genetic material from two women, something never sanctioned in humans, and would create mitochondrial changes that could be heritable (in female offspring), and therefore passed on in perpetuity. The manipulation would be performed on eggs or embryos, would affect every cell of the resulting individual, and once carried out this genetic manipulation is not reversible. Mitochondrial Replacement Techniques considers the implications of manipulating mitochondrial content both in children born to women as a result of participating in these studies and in descendants of any female offspring. This study examines the ethical and social issues related to MRTs, outlines principles that would provide a framework and foundation for oversight of MRTs, and develops recommendations to inform the Food and Drug Administration's consideration of investigational new drug applications.

Religiosity was thought to be a ‘natural’ part of femininity and turned religious masculinity into an oddity. This clear-cut gender ideology, however, remains an ideology (prescribed and contested) that needs to be put in the perspective of its context of origin, the bourgeois milieu. How were these gender identities constructed and by whom? With this volume Van Osselaer seeks to clarify how the gender differentiation was created among Belgian Catholics. She brings to light the extent to which religiosity was inscribed in these constructions and how religious teachings contributed to it. It is clear that the limitations of the ‘feminization’ thesis, a master narrative that has strongly contributed to the introduction of women in religious history, have gradually become more visible. Documenting pastoral care, the devotion to the Sacred Heart of Jesus and Catholic Action, The Pious Sex offers critical commentaries on the master narratives, suggesting that even men could belong to a ‘pious sex’.
