Book Lung Metastases and Isolated Lung Perfusion

Download or read book Lung Metastases and Isolated Lung Perfusion written by P. Van Schil and published by Nova Publishers. This book was released on 2007 with total page 254 pages. Available in PDF, EPUB and Kindle. Book excerpt: Surgical resection of lung metastases is a widely accepted procedure but long-term results are disappointing with a 5-year survival rate of approximately 40%. The main prognostic factor is complete resection and a better survival is reported in patients with a single metastasis and a disease-free survival of more than 3 years. Re-operations are feasible but often patients become inoperable due to insufficient pulmonary reserve and new treatment modalities are looked for. Intravenous (iv) chemotherapy has no major impact on survival due to systemic side-effects when high doses are given. As isolated limb and liver perfusion, isolated lung perfusion (ILuP) has the advantage of selectively delivering an agent into the lung while diverting the venous effluent. This allows the drug to be given in a significantly higher dose compared to iv therapy, while drug levels in the other organs are kept low enough to avoid major complications. ILuP has been shown to be very effective for the treatment of lung metastases in animal models. Phase I clinical studies to determine dose-limiting toxicity and maximum tolerated dose have recently been reported. The principal aim of this book is to discuss the current treatment of lung metastases and the possible contribution of ILuP.

  • Author : Karl Reinhard Aigner
  • Publisher : Springer
  • Release : 2016-05-27
  • ISBN : 3319287737
  • Pages : 501 pages

Download or read book Induction Chemotherapy written by Karl Reinhard Aigner and published by Springer. This book was released on 2016-05-27 with total page 501 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book presents the full range of management techniques and practices used in induction chemotherapy – both systemic and locoregional - by experienced international groups within one accessible volume. It provides the latest information on the pioneering and cutting-edge practices employed in different institutions and documents the advantages of integrated treatment schedules. All those responsible for treating patients with locally advanced cancers may find new, interesting aspects of locoregional cancer management and will find this book to be an invaluable source of up-to-date information. Throughout the world, induction chemotherapy is being used as part of an integrated plan of management for locally advanced, solid tumors, while systemic chemotherapy is most commonly used to shrink primarily non-operable tumors. In cases of poorly or nonresponsive tumors, attempts have been made to administer chemotherapeutics via the arterial route in order to achieve higher localized drug exposure and response rates. Such therapies are being practiced worldwide in independent, remote departments in universities, teaching hospitals and clinical schools. Frequently, however, these teams remain relatively unaware of the work being done in other institutions, which may slow further progress.

Book Locoregional Tumor Therapy

Download or read book Locoregional Tumor Therapy written by Eric Van Cutsem and published by Springer. This book was released on 2014-11-10 with total page 163 pages. Available in PDF, EPUB and Kindle. Book excerpt: Recent years have witnessed a renaissance in the use of locoregional tumor therapy as part of a multimodal therapeutic approach to a variety of solid tumors, and attempts are underway to define its value in the overall therapeutic strategy for individual tumor entities. This flourishing of interest in locoregional tumor therapy derives from a series of important new developments concerning methodology, drug treatments, and instrumentation. The resulting approaches include not only optimized locoregional chemotherapy and chemoembolization for hepatocellular carcinoma and other tumors, but also other “targeted” therapies such as stereotactic radiosurgery, radiofrequency ablation, intra-arterial administration of radiopharmaceuticals, and complex new methods of chemoembolization of liver metastases involving the use of multiagent chemotherapy in combination with targeted drugs, drug-eluting beads/microspheres, or nanoparticles. In addition, intraperitoneal and intrapleural therapies, e.g., cytoreduction plus hyperthermic intraperitoneal chemotherapy, are now a standard element of the strategy for a range of tumors. It is vital that practitioners have the necessary expertise in the use of these locoregional treatment methods and understand how to combine them optimally with modern systemic therapies. With that goal in mind, this book presents, in condensed form, the clinical results achieved using various locoregional tumor therapies. In so doing, it will allow the reader to rapidly retrieve the information on indications and effectiveness that is required for optimal integration of these highly effective therapies into modern, yet more complex treatment strategies.

Book Extreme Hepatic Surgery and Other Strategies

Download or read book Extreme Hepatic Surgery and Other Strategies written by Eduardo de Santibañes and published by Springer. This book was released on 2017-07-26 with total page 383 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book describes the diagnoses; staging and management of patients with colorectal liver metastases initially considered unresectable and portrays the different strategies to increase resectability along with their tactics and tricks. Colorectal carcinoma is the third most commonly diagnosed cancer in the world and according to recent cancer statistics around 1.23 million patients are diagnosed each year. Of these patients, approximately 50% will develop liver metastases during the course of their disease and around 15-25% are found to have stage IV disease at diagnosis. Liver resection has been recognized as the treatment of choice for these patients, offering overall 5-year survival rates of up to 50-60% and the only hope for cure. However, at diagnosis only 10-20% of these patients are possibly amenable to surgical resection with curative intent. The possibility to achieve an R0 resection is many times limited by the amount and quality of the future liver remnant (FLR), being posthepatectomy liver failure (PHLF) the most feared and severe complication after major liver resections. With the years, diverse strategies have been developed with the intention to increase resectability by increasing the future liver remnant and/or reducing tumor size, e.g. ALPPS. Along with these techniques, associated surgeries are developed including multivisceral resections, which broadens even more the resectability for patients.
