Book excerpt: Epileptic disorders need treatment for many years or even for life, and this makes a thorough understanding of the pharmacokinetics and possible hazards and side effects of the drugs used in treatment mandatory. During recent decades our knowledge in this field has considerably increased, not least as a result of the development of specific and sensitive methods for the determination of anti epileptic agents in biological material. The clinical pharmacology of this group of drugs has been studied extensively and can today be regarded as well established. This does not necessarily mean that drug treatment of epilepsy is without problems. For example, it has recently been shown that one of the newer anti epileptic drugs, greeted with great enthusiasm by clinicians, may in rare instances induce serious damage to the liver and the pancreas, and seems even to have a certain teratogenic potential. Clinical problems should be understood as a challenge to the experimental pharmacologist, who should try to find explanations for the clinical hazards, and, if possible, show new ways in which better drugs might be developed. In recent years interest has focused on the importance of the inhibitory transmitter 'l'-aminobutyric acid (GABA) in the pathophysiology of epilepsy, and there have been a series of attempts to find useful antiepileptic drugs among substances interfering with GABA metabolism in the CNS.

This is compounded by the increase of alcohol abuse, recently on the rise even in the developing countries. Concern with envi ronmental pollution has also raised the interest in liver diseases, in part because the liver acts as a guardian against polluting chemicals and in part because it is considered, possibly to an exaggerated degree, a vulnerable target of such chemicals.

Richardson called alcohol 'the most abused drug in the United States'. The report revealed that nine million Americans are alcohol abusers and that alcoholic individuals represent almost 10 % of the nation's work force. With spreading alcoholism, the incidence of physical damage due to alcohol has greatly increased. A question which is often raised is 'in which way does an alcoholic differ from a non-alcoholic?' Inquiries have focused on psychological make-up, behavioural differences and socioeconomic factors. More recently, however, physical differences have been delineated. Prior to the development of various disease entities, chronic ethanol exposure results in profound biochemical and morphological changes. Consequently an alcoholic does not respond normally to alcohol, or other drugs or even other toxic agents. Some of these persistent biochemical and morphological changes are the consequences of the injurious effects of ethanol, whereas others may represent the possible adaptive responses to the profound changes in intermediary metabolism which are a direct and im mediate consequence of the oxidation of ethanol itself. Differentiation between the effects of ethanol directly linked to its oxidation, and the adaptive and injurious effects of ethanol are not simple, and overlap is common. In general, however, metabolic effects are associated with the presence of relatively low ethanol concentrations, whereas injurious effects occur with high ethanol concentrations and/or after prolonged intake. High ethanol con centrations also produce so-called pharmacological effects.
