La crise suicidaire chez l'adolescent semble trouver ses sources, chez des adolescents « fragiles », dans une difficulté à métaboliser les bouleversements vécus lors de la crise de l'adolescence, à s'autonomiser. Dans une perspective systémique, la crise de l'adolescence, en faisant entrer la famille dans une nouvelle étape de son cycle de vie, est une crise pour le système familial tout entier, impliquant des changements très importants. Cette crise est censée être un moment positif, puisque les conflits qu'elle induit permettent de faire évoluer la famille vers une nouvelle phase. C'est lorsque la famille est bloquée dans un non-changement, que les informations et les ressentis ne peuvent être partagés, que se pose le problème de la crise suicidaire pour l'adolescent. Elle est alors à la fois une tentative de « non-changement » et d'un premier pas vers une demande d'aide extérieure. Dans ce contexte, la thérapie familiale, qui a prouvé son efficacité dans de nombreux autres contextes, semble trouver sa place. Le modèle proposé par Jurich, notamment, nous a paru intéressant par son intégrabilité et son adaptabilité, et son travail de développement des compétences familiales. Il serait intéressant de l'évaluer.

Les taux de décès par suicide et de tentatives de suicide (TS) augmentent au cours de l'adolescence et le suicide est la principale cause de décès chez les adolescents. Nous avons pu voir dans notre revue de la littérature que les recommandations au niveau international manquent de consensus. La littérature sur la prise en charge des adolescents suicidants, bien que de plus en plus fournie, manque d'études aux méthodologie robustes. Objectif : Notre étude se déroule sur deux ans et a pour but d'évaluer les pratiques professionnelles de la prise en charge des adolescents suicidants hospitalisés au CH de Saintes, incluant le protocole de thérapie familiale. Il s'agit d'étudier le devenir de ces patients à un an selon trois axes: taux de suivi, taux de récidives et un axe clinique. Méthode : Nous avons inclus 23 adolescents suicidants, âgés de 12 à 17 ans, hospitalisés dans le service de pédiatrie ou de pédopsychiatrie du CH de Saintes, sur la période allant de juin 2015 à juin 2016. Nous les avons ensuite contactés par téléphone à 3 mois, 6 mois et 12 mois de leur sortie d'hospitalisation pour recueillir les différents éléments cliniques et du suivi. Résultats : Le résultat le plus important de notre étude est le taux d'adhésion au protocole de thérapie familiale (82,3% à 3 mois) et cela pourrait être une piste intéressante dans la prévention des récidives. De plus, on observe au bout des douze mois de suivi une diminution des scores du risque suicidaire qui passent de 7 sur 20 en moyenne à l'admission à 2 sur 20 à 12 mois. Conclusion : Malgré quelques données intéressantes, notre étude reste limitée et ne permet pas de conclure compte tenu de son faible effectif et de l'absence de groupe contrôle. La prise en charge des adolescents suicidants reste un enjeu majeur en psychiatrie et plus globalement en santé publique. En effet, étant donné que le suicide est une cause majeure de décès et de handicap, la mise en œuvre de stratégies efficaces, fondées sur des données probantes est l'obligation et la responsabilité des décideurs et des hautes instances de santé.

It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments?
