Book excerpt: It is unknown whether the application of an ankle brace can increase dynamic stability to overcome the effects of fatigue in those individuals with CAI. Ankle dynamic stability was assessed before and after a functional fatigue protocol using the Time to Stabilization (TTS) method. Prior to the functional task, dynamic stability was measured using a single leg jump landing at 50% of the subjects' maximum vertical jump height. The functional fatigue circuit was repeated until the time it took to complete the circuit increased to 50% of the baseline time. Immediately after fatigue was reached, the single leg jump landing measures were repeated. This protocol was performed with and without an ankle brace. For APTTS, the main effect for Group was statistically significant (F= 4.15; p= .05). Subjects in the CAI group (1.291"0.179s) took significantly longer to stabilize than those in the healthy group (1.191"0.10s). There were no other significant results found. The results of our study provide questions regarding the use of ankle braces for improving dynamic stability measured with TTS in both a healthy population and those with CAI. Further research, with a larger sample size should be conducted to make a better decision regarding the effectiveness of ankle braces in improving single-leg jump landing stabilization.

Book excerpt: The purpose of this study was to determine if prophylactic ankle tape and/or ankle braces improve dynamic stability in TTS measure. All subjects were healthy and had no prior history of ankle injuries. Data collection consisted of each subject performing a single leg jump-landing with ankle tape, ankle brace, combination of the two, and control (no tape or brace) conditions. Dynamic stability was assessed with time to stabilization force plate measure. Significant plane by ankle tape interaction (p=0.045) was found. No significant plane by ankle tape by ankle brace interaction (p=0.637), no significant ankle tape by ankle brace interaction (p=0.483), or plane by ankle brace interaction (p=0.697) were found. A notable finding was that subjects took longer to stabilize in the anterior/posterior direction than medial/lateral direction. In conclusion ankle tape, ankle brace, and the combination of ankle tape and ankle brace did not statistically improve dynamic stability in healthy ankles.

Book excerpt: Musculoskeletal injuries sustained during physical training and sports-related activities are the greatest threat to military force readiness. Injuries sustained during physical training are one of the principal reasons for the inability of service men and women to be deployed and for the almost 25 million limited duty days accrued annually. Ankle sprains have been shown to be a leading cause for military disability and are a long-term health care issue, potentially leading to a medical discharge. Military parachuting studies have demonstrated the benefits of using an outside-the-boot ankle brace to reduce the risk for sustaining an ankle injury, but the use of ankle bracing on other performance measures in the military have not been established. The first chapter of this dissertation focused on reviewing existing literature that investigated the effects of ankle bracing on functional performance measures in civilian populations. It was concluded in the literature that ankle braces, used in healthy physically active populations, have little statistical impact on balance and functional performance measures. The second chapter of this dissertation focused on reviewing how ankle braces effect performance measures in military specific populations. At this point, only one study has specifically evaluated how these devices affect performance measures, and the remaining studies focused only on the reduction of ankle injuries. Due to the lack of literature available, the purpose of this study was to investigate the effects of ankle braces on dynamic balance and three functional performance measures in a Reserve Officers Training Corps. This randomized crossover study indicated significant results in which ankle braces do create negative effects for balance reach distances, max jump height, max jump distance, and obstacle course completion time. This is the first study to evaluate ankle braces in a military sample while wearing battle dress uniform, combat boots, and a loaded rucksack.

The efficacy of the ankle taping for injury prevention has long been under scrutiny as numerous studies have shown that tape rapidly loses its ability to constrain ankle motion with exercise. Consequently, ankle braces (orthoses) are being used with increasing frequency for the prevention and functional management of ankle injuries. However, the motion restraining qualities of ankle orthoses have not been widely evaluated in closed kinetic chain environments under physiologic loads. The primary purpose of this study was to compare the abilities of four ankle orthoses (ankle taping, lace-up brace, semirigid orthosis and hybrid brace) against a control condition (no brace or tape) to control subtalar and talocrural motion during running on a laterally-tilted treadmill at 16.2 km/h before and after exercise. It has been hypothesized that ankle orthoses make a secondary contribution to injury prevention through enhanced proprioception. The secondary purpose of this study was to quantify the effects of the aforementioned ankle orthoses on postural stability during single-limb stance following a bout of exercise. Fifteen healthy university students (8 men and 7 women) with no history of significant ankle injuries (age, mean ± SD: 22.9 ± 3.9 years) volunteered to participate in this study. Three-dimensional kinematic data were captured with an active infrared digital camera system sampling at 120 Hz. To address the first question, data analyses were performed using 2way univariate (Ankle Orthoses x Pre/Post-Exercise x Subjects) (5 x 2 x 15) repeated measures analysis of variance (ANOVA) to determine the existence of differences among three closed and four open kinematic chain dependent measures before and after exercise. Maximum inversion angles (MAXINV) were similar for all ankle orthoses, with no orthosis limiting inversion during tilted treadmill running significantly more than another, or compared to the control condition, either before or after exercise (p>.05). Pre-exercise MAXINV group means and standard deviations during treadmill running ranged from 6.8 ± 3.4 deg with the Royce Medical Speed Brace to 9.5 ± 4.1 deg in the tape condition; post-exercise MAXINV mean values ranged from 7.6 ± 3.2 deg for the Aircast Sport Stirrup to 9.1 ± 4.6 deg with closed basketweave tape. While not statistically significant (p=0.10), ankle taping provided the least amount of inversion restraint, both before and after the exercise bout. The MAXINV angles measured during treadmill running (8.2 ± 4.0 deg) and open chain inversion AROM measured with a goniometer (34.5 ± 6.2 deg) were not related (r=-0.0003). The compressive forces present during closed kinetic chain activity are known to increase joint stability and thus may explain why MAXINV under dynamic varus loads was so much less in magnitude than inversion AROM measured under open kinetic chain conditions. The nonlinear relationship of these two variables supports our contention that reports of the motion controlling properties of ankle orthoses measured in open kinetic chain environments should not be used to infer the response characteristics of these same orthoses under dynamic, physiologic loads. To address the second question, data were analyzed using 3-way univariate (Ankle Orthoses x Pre/Post-Exercise x Eyes Open/Closed x Subjects) (5 x 2 x 2 x 15) repeated measures ANOVAs. Subjects' postural stability was assessed using a Biodex Balance System with eyes open and eyes closed conditions, before and after an exercise bout. The ankle orthoses evaluated did not influence postural stability as measured by mediolateral sway index, anteroposterior sway index, and overall sway index. Removal of visual perception via blindfolding resulted in significant decreases in all three measures of postural stability (p=.001). There was poor association among the closed chain postural stability parameters and the open chain AROM measures. These correlations ranged from r=.04 to .17, indicating minimal relationship between the amount of AROM permitted by the orthoses and postural stability as quantified by this method.

They are intended to restrict excessive ankle joint motion; however, their effectiveness during sport activity is inconclusive. Objective: To compare differences in laxity at the ankle in taped and braced conditions, in individuals with and without ankle instability, before and after exercise. Design: Pre-test post-test design with control group. Setting: Human Performance Laboratory. Participants: Twenty-four participants (20.6±1.6yrs, 173.6±8.3cm, 72.8±12.2kg) were placed into one of three groups; ankle instability, previous sprain with no instability, or no previous sprain. Intervention(s): Ankle laxity was assessed on each subject to test the effect of an ankle taping and bracing from pre- to post- exercise. A 20-minute exercise protocol was performed utilizing each of the external prophylactic support (EPS) conditions (tape, brace, control/none). Main Outcome Measure(s): Ankle laxity (anterior displacement and inversion-eversion rotation) was measured using an ankle arthrometer under three conditions and in three groups. Book excerpt: Abstract: Background The application of ankle braces is an effective method for the prevention of recurrent ankle sprains. It has been proposed that the reduction of injury rates is based on the mechanical stiffness of the brace and on beneficial effects on proprioception and neuromuscular activation. Yet, how the neuromuscular system responds to the application of various types of ankle braces during highly dynamic injury-relevant movements is not well understood. Enhanced stability of the ankle joint seems especially important for people with chronic ankle instability. We therefore aimed to analyse the effects of a soft and a semi-rigid ankle brace on the execution of highly dynamic 180° turning movements in participants with and without chronic ankle instability. Methods Fifteen participants with functional ankle instability, 15 participants with functional and mechanical ankle instability and 15 healthy controls performed 180° turning movements in reaction to light signals in a cross-sectional descriptive laboratory study. Ankle joint kinematics and kinetics as well as neuromuscular activation of muscles surrounding the ankle joint were determined. Two-way repeated measures analyses of variance and post-hoc t-tests were calculated. Results Maximum ankle inversion angles and velocities were significantly reduced with the semi-rigid brace in comparison to the conditions without a brace and with the soft brace (p ≤ 0.006, d ≥ 0.303). Effect sizes of these reductions were larger in participants with chronic ankle instability than in healthy controls. Furthermore, peroneal activation levels decreased significantly with the semi-rigid brace in the 100 ms before and after ground contact. No statistically significant brace by group effects were found. Conclusions Based on these findings, we argue that people with ankle instability in particular seem to benefit from a semi-rigid ankle brace, which allows them to keep ankle inversion angles in a range that is comparable to values of healthy people. Lower ankle inversion angles and velocities with a semi-rigid brace may explain reduced injury incidences with brace application. The lack of effect of the soft brace indicates that the primary mechanism behind the reduction of inversion angles and velocities is the mechanical resistance of the brace in the frontal plane
