Book excerpt: Globalization of the food supply has created conditions favorable for the emergence, reemergence, and spread of food-borne pathogens-compounding the challenge of anticipating, detecting, and effectively responding to food-borne threats to health. In the United States, food-borne agents affect 1 out of 6 individuals and cause approximately 48 million illnesses, 128,000 hospitalizations, and 3,000 deaths each year. This figure likely represents just the tip of the iceberg, because it fails to account for the broad array of food-borne illnesses or for their wide-ranging repercussions for consumers, government, and the food industry-both domestically and internationally. A One Health approach to food safety may hold the promise of harnessing and integrating the expertise and resources from across the spectrum of multiple health domains including the human and veterinary medical and plant pathology communities with those of the wildlife and aquatic health and ecology communities. The IOM's Forum on Microbial Threats hosted a public workshop on December 13 and 14, 2011 that examined issues critical to the protection of the nation's food supply. The workshop explored existing knowledge and unanswered questions on the nature and extent of food-borne threats to health. Participants discussed the globalization of the U.S. food supply and the burden of illness associated with foodborne threats to health; considered the spectrum of food-borne threats as well as illustrative case studies; reviewed existing research, policies, and practices to prevent and mitigate foodborne threats; and, identified opportunities to reduce future threats to the nation's food supply through the use of a "One Health" approach to food safety. As a worldwide economic downturn has gathered pace, commodity markets have weakened significantly. By October 2008, the UN Food and Agriculture Organization's Food Price Index stood at 164, the same level as in August 2007, and 25% lower than the Index's high of 219 in June 2008. However, this does not mean that policy-makers around the world can start to breathe a sigh of relief. For one thing, even at their somewhat diminished levels current prices remain acutely problematic for low-income import-dependent countries and for poor people all over the world. The World Bank estimates that higher food prices have increased the number of undernourished people by as much as 100 million from its pre-price-spike level of 850 million.

Book excerpt: New evidence this year corroborates the rise in world hunger observed in this report last year, sending a warning that more action is needed if we aspire to end world hunger and malnutrition in all its forms by 2030. Updated estimates show the number of people who suffer from hunger has been growing over the past three years, returning to prevailing levels from almost a decade ago. Although progress continues to be made in reducing child stunting, over 22 percent of children under five years of age are still affected. Other forms of malnutrition are also growing: adult obesity continues to increase in countries irrespective of their income levels, and many countries are coping with multiple forms of malnutrition at the same time – overweight and obesity, as well as anaemia in women, and child stunting and wasting.

While noting that there is always more to learn and that the scientific process is never closed, the book shows that hypotheses about climate change are supported by multiple lines of evidence and have stood firm in the face of serious debate and careful evaluation of alternative explanations. As decision makers respond to these risks, the nation's scientific enterprise can contribute through research that improves understanding of the causes and consequences of climate change and also is useful to decision makers at the local, regional, national, and international levels. The book identifies decisions being made in 12 sectors, ranging from agriculture to transportation, to identify decisions being made in response to climate change. Advancing the Science of Climate Change calls for a single federal entity or program to coordinate a national, multidisciplinary research effort aimed at improving both understanding and responses to climate change. Seven cross-cutting research themes are identified to support this scientific enterprise. In addition, leaders of federal climate research should redouble efforts to deploy a comprehensive climate observing system, improve climate models and other analytical tools, invest in human capital, and improve linkages between research and decisions by forming partnerships with action-oriented programs.

Book excerpt: Updates for many countries have made it possible to estimate hunger in the world with greater accuracy this year. In particular, newly accessible data enabled the revision of the entire series of undernourishment estimates for China back to 2000, resulting in a substantial downward shift of the series of the number of undernourished in the world. Nevertheless, the revision confirms the trend reported in past editions: the number of people affected by hunger globally has been slowly on the rise since 2014. The report also shows that the burden of malnutrition in all its forms continues to be a challenge. There has been some progress for child stunting, low birthweight and exclusive breastfeeding, but at a pace that is still too slow. Childhood overweight is not improving and adult obesity is on the rise in all regions. The report complements the usual assessment of food security and nutrition with projections of what the world may look like in 2030, if trends of the last decade continue. Projections show that the world is not on track to achieve Zero Hunger by 2030 and, despite some progress, most indicators are also not on track to meet global nutrition targets. The food security and nutritional status of the most vulnerable population groups is likely to deteriorate further due to the health and socio economic impacts of the COVID-19 pandemic. The report puts a spotlight on diet quality as a critical link between food security and nutrition. Meeting SDG 2 targets will only be possible if people have enough food to eat and if what they are eating is nutritious and affordable. The report also introduces new analysis of the cost and affordability of healthy diets around the world, by region and in different development contexts. It presents valuations of the health and climate-change costs associated with current food consumption patterns, as well as the potential cost savings if food consumption patterns were to shift towards healthy diets that include sustainability considerations. The report then concludes with a discussion of the policies and strategies to transform food systems to ensure affordable healthy diets, as part of the required efforts to end both hunger and all forms of malnutrition.

Production increase is based on area expansion and output per hectare, and reflects changing crop patterns as well as new technology in fertilization and irrigation. Approaches to increasing the world food supply must take into account physical and economic conditions and potential problems, such as soil management.

Book excerpt: The U.S. veterinary medical profession contributes to society in diverse ways, from developing drugs and protecting the food supply to treating companion animals and investigating animal diseases in the wild. In a study of the issues related to the veterinary medical workforce, including demographics, workforce supply, trends affecting job availability, and capacity of the educational system to fill future demands, a National Research Council committee found that the profession faces important challenges in maintaining the economic sustainability of veterinary practice and education, building its scholarly foundations, and evolving veterinary service to meet changing societal needs. Many concerns about the profession came into focus following the outbreak of West Nile fever in 1999, and the subsequent outbreaks of SARS, monkeypox, bovine spongiform encephalopathy, highly pathogenic avian influenza, H1N1 influenza, and a variety of food safety and environmental issues heightened public concerns. They also raised further questions about the directions of veterinary medicine and the capacity of public health service the profession provides both in the United States and abroad. To address some of the problems facing the veterinary profession, greater public and private support for education and research in veterinary medicine is needed. The public, policymakers, and even medical professionals are frequently unaware of how veterinary medicine fundamentally supports both animal and human health and well-being. This report seeks to broaden the public's understanding and attempts to anticipate some of the needs and measures that are essential for the profession to fulfill given its changing roles in the 21st century.

The global financial crisis that began in 2008 compounded the burden of high food prices, exacerbating the problems of hunger and malnutrition in developing countries. The tandem food price and economic crises struck amidst the massive, chronic problem of hunger and undernutrition in developing countries. National governments and international actors have taken a variety of steps to mitigate the negative effects of increased food prices on particular groups. The recent abrupt increase in food prices, in tandem with the current global economic crisis, threatens progress already made in these areas, and could inhibit future efforts. The Institute of Medicine held a workshop, summarized in this volume, to describe the dynamic technological, agricultural, and economic issues contributing to the food price increases of 2007 and 2008 and their impacts on health and nutrition in resource-poor regions. The compounding effects of the current global economic downturn on nutrition motivated additional discussions on these dual crises, their impacts on the nutritional status of vulnerable populations, and opportunities to mitigate their negative nutritional effects.
