When coupled with appropriate treatment, early detection can reduce breast cancer mortality. At the request of Congress, the Food and Drug Administration (FDA) commissioned a study to examine the current practice of mammography and breast cancer detection, with a focus on the FDA's oversight via the Mammography Quality Standards Act (MQSA), to identify areas in need of improvement. Enacted in 1993, MQSA provides a general framework for ensuring national quality standards in facilities performing screening mammography, requires that each mammography facility be accredited and certified, and mandates that facilities will undergo annual inspections. This book recommends strategies for achieving continued progress in assuring mammography quality, including changes to MQSA regulation, as well as approaches that do not fall within the purview of MQSA. Specifically, this book provides recommendations aimed at improving mammography interpretation; revising MQSA regulations, inspections, and enforcement; ensuring an adequate workforce for breast cancer screening and diagnosis; and improving breast imaging quality beyond mammography.

In 2004, 186,772 women were diagnosed with breast cancer and 40,954 women died of breast cancer. According to the National Cancer Institute, there is a strong association among breast cancer screening, early detection, and survival. There is a marked deficit of breast cancer screening adherence. Early detection of breast cancer with a corresponding decrease in mortality is dependent on breast cancer screening. This research was a non-experimental, descriptive, retrospective, quantitative chart review. The purpose of this study was to determine if participants at this study site, who received counseling on breast cancer screening, adhered to the protocol to make changes in preventive health. The Health Belief Model was the organizing framework for this study. This framework was used to organize this research study on adherence to breast cancer screening guidelines because it focuses on patient adherence and preventive healthcare practice. The sample of charts were of women age 40 years or over, who had no insurance, and were below 250% of the federal poverty level who presented at this study site for services. Data were collected from 103 charts of patients that received services from January 2007 to January 2008. Information on patients' age, ethnicity, race, adherence to breast self exam, adherence to clinical breast exam, and adherence to mammography were collected. The results showed an impressive adherence to mammogram, followed by BSE, then CBE. The study suggested that tailored interventions pertaining to performing monthly BSE and yearly CBE with mammography has the potential to make a positive difference in breast cancer screening behaviors.

Book excerpt: Women living with HIV (WLWH) in the U.S. are more than twice as likely to die of breast cancer compared to women without HIV. Although WLWH are less likely to obtain mammography than women without HIV, research examining risk factors for non-adherence to screening among WLWH is limited, particularly among those aged 50-74 years for whom biennial screening is currently recommended. This project examined risk factors for non-adherence to screening mammography among WLWH aged 50-74 receiving primary care at UC San Diego and UC San Francisco between 2016 and 2018. This study used electronic medical record data to compare rates of adherence to mammography guidelines between WLWH and the general population of women in California (Aim 1). Logistic regression analyses were conducted to evaluate individual (i.e., age, race/ethnicity, medical and psychiatric co-morbidities, smoking status, HIV-related medical characteristics), interpersonal (i.e., marital status, PCP department, PCP relationship length, number of office visits), and organizational (i.e., insurance, healthcare site) factors associated with non-adherence to recommended mammography screening among WLWH (Aims 2, 3, 4). Multiple logistic regression was used to identify the unique contribution of variables identified in Aims 2, 3, and 4 in predicting non-adherence to screening mammography and to determine the influence of individual, interpersonal, and organizational variable(s) on model fit (Aim 5). WLWH (N = 293, mean age = 57.4, 36.2% non-Hispanic White) were significantly less likely to obtain mammography screening than the general population. WLWH at greatest risk for non-adherence to mammography screening included those with mental health or substance use diagnoses, Hispanic women, and women who engaged in fewer office visits. In adjusted analyses, WLWH with a mental health diagnosis were more than twice as likely to be non-adherent with screening recommendations than their peers, and Hispanic women were twice as likely to be non-adherent than non-Hispanic White women. The inclusion of interpersonal, but not organizational, variables significantly improved model fit. Evidence-based interventions are needed to increase adherence to mammography screening among WLWH. Such interventions have the potential to increase the uptake of timely breast cancer screening and decrease breast-cancer related morbidity and mortality among this medically vulnerable population.

Due to the combination of improved treatments and the benefits of mammography screening, breast cancer mortality has decreased steadily since 1989. Yet breast cancer remains a major problem, second only to lung cancer as a leading cause of death from cancer for women. To date, no means to prevent breast cancer has been discovered and experience has shown that treatments are most effective when a cancer is detected early, before it has spread to other tissues. These two facts suggest that the most effective way to continue reducing the death toll from breast cancer is improved early detection and diagnosis. Building on the 2001 report Mammography and Beyond, this new book not only examines ways to improve implementation and use of new and current breast cancer detection technologies but also evaluates the need to develop tools that identify women who would benefit most from early detection screening. Saving Women's Lives: Strategies for Improving Breast Cancer Detection and Diagnosis encourages more research that integrates the development, validation, and analysis of the types of technologies in clinical practice that promote improved risk identification techniques. In this way, methods and technologies that improve detection and diagnosis can be more effectively developed and implemented.

Available in PDF, EPUB and Kindle. Book excerpt: Background: Cancer screening guidelines reflect the costs and benefits of population-based screening based on evidence from clinical trials. While most of the existing literature on compliance with cancer screening guidelines only measures raw screening rates in the target age groups, we used a novel approach to estimate degree of guideline compliance across Canadian provinces for breast, colorectal and prostate cancer screening. Measuring compliance as the change in age-specific screening rates at the guideline-recommended initiation age (50), we generally found screening patterns across Canadian provinces that were not consistent with guideline compliance. Methods: We calculated age-cancer-specific screening rates for ages 40-60 using the Canadian Community Health Survey (2003 and 2005), a cross-sectional, nationally representative survey of health status, health care utilization and health determinants in the Canadian population. We estimated the degree of compliance using logistic regression to measure the change in adjusted screening rates at the guideline-recommended initiation age for each province in the sample. Results: For breast cancer, after adjusting for age trends and other covariates, being above age 50 in Quebec increased the probability of being screened by 19 percentage points, from an average screening rate of 24% among 40-49 year olds. None of the other regions exhibited a statistically significant change in screening rates at age 50. Additional analyses indicated that these patterns reflect asymptomatic screening and that Quebec's breast cancer screening program enhanced the degree of guideline compliance in that province. Colorectal cancer screening practice was consistent with guidelines only in Saskatchewan, as screening rates increased at age 50 by 12 percentage points, from an average rate of 6% among 40-49 year olds. For prostate cancer, the regions examined here are not compliant with Canadian guidelines since screening rates were quite high, and there was not a discrete increase at any particular age. Conclusions: Screening practice for breast, colorectal and prostate cancer was generally not consistent with Canadian clinical guidelines. Quebec (breast) and Saskatchewan (colorectal) were exceptions to this, and the impact of Quebec's breast cancer screening program suggests a role for policy in improving screening guideline compliance.
