Book excerpt: This paper analyzes the extent of income inequality from a global perspective, its drivers, and what to do about it. The drivers of inequality vary widely amongst countries, with some common drivers being the skill premium associated with technical change and globalization, weakening protection for labor, and lack of financial inclusion in developing countries. We find that increasing the income share of the poor and the middle class actually increases growth while a rising income share of the top 20 percent results in lower growth—that is, when the rich get richer, benefits do not trickle down. This suggests that policies need to be country specific but should focus on raising the income share of the poor, and ensuring there is no hollowing out of the middle class. To tackle inequality, financial inclusion is imperative in emerging and developing countries while in advanced economies, policies should focus on raising human capital and skills and making tax systems more progressive.

Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

Book excerpt: We consider the changes in BC inequality in a number of different ways: broad trends using the gini coefficient (the most common summary index of inequality); changes in incomes for different income groups; the shares of income accruing to each income group; and, the ratio of incomes at the top, bottom and middle of the distribution. [...] A large part of the increase in inequality stems from the near-collapse of incomes at the bottom of the distribution. [...] If changes in inequality appear more at the top or bottom of the distribution, they may not be adequately captured by the gini.3 This paper also examines changes in the average income of different income groups over time, and the ratio of the average income at the top, bottom and middle of the distribution. [...] For the bottom five vingtiles (25% of the population), the absolute decline in income (not shown) was greater after taxes and transfers than it was for market income.7 To further illustrate the changes over time, Figure 4a shows that the changes in average income for the bottom vingtile are not aberrations for the year 2000. [...] As a result of the gains at the top and losses at the bottom, the ratio of market incomes of the top vingtile to the bottom vingtile grew dramatically over the 1992-2000 period.

In the late 1980s, a second measure, called the Low Income Measure (LIM), began gaining in popularity for certain types of analyses. Both are by design relative measures. Statistics Canada has maintained that neither measure is intended as a poverty line but, in the absence of other measures, they are at times interpreted as such. There is currently an initiative underway in Canada, sponsored by the provincial and territorial Ministers of Social Services and the federal Human Resources Development Canada to devise a needs-based measure of poverty called the Market Basket Measure (MBM). The note outlines the existing measures and summarizes the recent developments.
