Efficiency in health service provision requires that providers and consumers have incentives to use healthcare resources in ways that generate the maximum health gains. Equity in at least one sense requires that consumers requiring the same care are treated equally, irrespective of their ability to pay. Efficiency in the use of health services requires that consumers are knowledgeable about the services on offer and which are most appropriate to their needs. Although these principles are enshrined in the design of every health system in the world, they have proven extremely difficult to apply in practice. Healthcare providers have financial obligations to their families as well as professional obligations to their patients. Health service consumers generally lack information about both their health and health services so that they under-consume or over-consume healthcare. The papers in this volume are selected from an international conference organized by the CDRI, Cambodia, that tried to deal with some of these issues. With participation of international and local experts, it aimed at collecting major experiences and innovative solutions from inside and outside the country to improve health sector performance, with particular focus on institutions, motivations and incentives.

The challenge now is to sustain and enhance these achievements in a context of tight fiscal constraints in many countries combined with upward pressure on health spending from factors such as new technological advances and demographic changes. Finding policies that can make health spending more sustainable without compromising important achievements in access and quality requires effective co-operation between health and finance ministries. Sound governance and co-ordination mechanisms are therefore essential to ensure effective policy choices. Prepared by both public finance and health experts, this report provides a unique detailed overview of institutional frameworks for financing health care in OECD countries. If one peruses the index of documents discussed in Volume Four, certain topics are more often the focus of legislation and official concern than others: withholding and withdrawing treatment, access to health care, consent to treatment and experimentation, and issues posed by HIV testing and AIDS. Such a common focus should not be exaggerated, for the discussion of topics is wide-ranging. But that commonality, when in evidence, is also not surprising. It suggests that key issues and concerns in bioethics may be widely shared among modern cultures and societies, for all the distinctiveness of a particular nation's or region's response to them. Issues of informed consent, after all, implicate more fundamental matters of respect for persons and the rights of individuals in the contexts of therapy and research. Issues of access to medical care concretize deeper questions about the nature and scope of a society's welfare obligations to its citizens.
