Book excerpt: Introduction: Intervertebral disc (IVD) disorders are associated with neck and low back pain and accompanying disabilities. The most common symptomatic IVD disorder is degeneration of the disc, which is a multifactorial disease. Currently, the pathobiology of IVD degeneration is poorly understood, although aging, injury, nutrition, metabolism and mechanical stress are suspected to contribute. As such, in vitro models capable of replicating environmental factors thought to contribute to clinical degeneration would provide powerful tools for investigating disease onset and progression. Whole organ IVD models are capable of maintaining cell and tissue integrity, architecture, composition, and cellular viability while preserving biologic, metabolic, and biomechanical functions found in vivo. Additionally, the whole organ culture model allows for assessment of disc health through relevant biomarker analyses. This study was designed to assess the initial pathophysiologic and metabolic changes that occur as a result of injury and/or pro-inflammatory insult to rat tail whole organ IVDs in culture. It was hypothesized that injury and/or pro-inflammatory insult to the IVD would result in a significant increase in production of inflammatory mediators and degradative enzyme activity (MCP-1, IL-6, VEGF, Gro-u03b1, general MMP activity, and Nitric Oxide) compared to controls. Methods: tTissue harvest and culture: Under IACUC approval (ACUC#9435), tails were collected from 10 skeletally mature Sprague Dawley rats after they were euthanatized for reasons unrelated to this study. Soft tissues were dissected aseptically from the caudal vertebrae. Explants consisting of cranial vertebral body, cartilage endplate, IVD, cartilage endplate, and caudal vertebral body half were harvested (Figure 1). IVD whole organ explants were randomly assigned to either the Injured or Uninjured group (n=9/group). For the injured group, the posterolateral annulus fibrosis of each explant was penetrated with a 20G needle to enter the approximate center of the nucleus pulposus, which was then aspirated with a 1 mL syringe to 0.5 mL (n=45). Uninjured explants received no insult. The explants were then assigned to one of following nine culture groups based on cytokine stimulation: (1) No injury no cytokine (NC) (2) Injury no cytokine (IC) (3) High TNF-u03b1 (10ng/ml) + Injury (HTI) (4) High TNF-u03b1 (10ng/ml) - Injury (HTU) (5) Low TNF-u03b1 (0.1ng/ml) + Injury (LTI) (6) Low TNF-u03b1 (0.1ng/ml) - Injury (LTU) (7) High IL-1u03b2 (10ng/ml) + Injury (HII) (8) High IL-1u03b2 (10ng/ml) - Injury (HIU) (9) Low IL-1u03b2 (0.1ng/ml) + Injury (LII) (10) Low IL-1u03b2 (0.1ng/ml) - Injury (LIU) cultured for 3 days (n=9/group) using a 24 well plate in 2 mL of Dulbeccou2019s modified Eagleu2019s medium supplemented with sodium pyruvate, L-glutamine, ascorbic acid, MEM N-E Amino Acid solutions, ITS, and penicillin-streptomycin in a normal culture environment. Culture media were collected on day 3 for biomarker analyses. Media Analyses: Media from day 3 were analyzed for Nitric Oxide, general MMP activity, IL-6, MCP-1, Gro-u03b1, and VEGF using commercially available assays according to the manufactureu2019s protocol.
