Book excerpt: ABSTRACT: Background: Osteoporosis is a bone disease that causes decreased bone mass and bone density, increasing the risk of fracture and decreasing health-related quality of life (HRQoL). Strength training and other exercise interventions are often prescribed to treat low bone mineral density. The purpose of this review is to determine if strength and resistance training can decrease pain levels or increase HRQoL. Methods: A comprehensive search was performed in the following databases using a pre-selected string: PubMed, CINAHL, Trip, Pedro, EBSCO, and MEDLINE. The inclusion criteria consisted of: articles that used strength or resistive training methods as an intervention on postmenopausal women with either osteoporosis or osteopenia, and applied an outcome measure incorporating HRQoL and/or pain. Articles were excluded if participants presented with interfering health conditions, the exercise intervention did not include a strength training aspect, patients were being treated pharmacologically, with exceptions of calcium and vitamin D supplements, and if the articles were not written in English. Results: Six articles were included in this systematic review. Analysis of this literature indicates that strength training improves HRQoL and decreases pain in postmenopausal women with osteoporosis. Discussion/Conclusion: Little peer-reviewed evidence exists for strength training interventions in women with osteoporosis. All evidence supported interventions of at least 10 weeks in length, with the longest intervention spanning 2 years. Strength training alone was found to significantly decrease pain in 2 studies. Interventions combining strength training and other forms of exercise were found to increase HRQoL significantly more than strength training alone in women with osteopenia and osteoporosis.

Research articles were chosen that focused on HRQoL and Bone Mineral Density (BMD)in postmenopausal women with osteopenia/osteoporosis. Results: After reviews of each article, 16 studies met the inclusion criteria. The evidence suggests that exercise intervention (strengthening, balance training, fall prevention, aerobic, and aquatic exercise)can have positive effects on postmenopausal women with osteopenia/osteoporosis. Discussion: Exercise intervention should be a component of standard care for women diagnosed with osteopenia/osteoporosis. Clinical Implications: Clinical application of the findings of this systematic review can be used on a wide variety of patients. A patient with osteopenia/osteoporosis will benefit from exercise intervention and this can be performed without any extra cost." -- Abstract.

Abstract: Osteoporosis is a serious health problem worldwide and most commonly affects postmenopausal women (World Health Organization, 1994). Of the variety of complications associated with osteoporosis in postmenopausal women, osteoporotic fracture is the most prevalent and serious one. It affects their quality of life and puts heavy burden on the healthcare system in Hong Kong. Since osteoporosis exercise program can help to reduce bone loss, it seems to be an effective strategy to prevent and manage osteoporosis in postmenopausal women. The purpose of this study is to develop an evidence-based osteoporosis exercise education program for postmenopausal women, based on the best available evidence, with an aim of helping them to prevent and manage osteoporosis through the prevention and reduction of bone loss and its related fractures. A search of literature from four databases, including Pubmed (1997 to August 2011), CINAHL Plus (1937 to August 2011), Cochrane Library (1996 to Issue 8 of 2011) and EMBASE Classic + EMBASE (1947 to August 2011), was done to identify relevant randomized controlled trials (RCTs). The methodology checklist for RCTs developed by the Scottish Intercollegiate Guidelines Network (SIGN, 2008) was used to evaluate the quality of the selected studies. A total of nine studies met the inclusion criteria and did not violate the exclusion criteria. The findings of these studies showed that exercise intervention or program significantly reduced postmenopausal women's likelihood of having fracture and also improved their bone mass density slightly. Among the reviewed studies, programs were most commonly implemented within a 12-month period and two 60-minute supervised exercise sessions were delivered per week. The potential of implementing the proposed program, based on the evidence from the literature review, to local practice was assessed. Based on the assessment, the proposed program is necessary and beneficial for postmenopausal women. An evidence-based osteoporosis exercise education program is developed based on the best available evidence. The SIGN grading system was utilized to provide grades for the recommendations in the guideline. An effective and adequate communication with potential stakeholders to gain their support and cooperation for the proposed program is essential. In addition, a tailor-made pilot test is also crucial for ensuring the successful implementation of the proposed program. The pilot study will be conducted in a local practice and the targeted participants are 124 postmenopausal women. Data analysis will be done by the paired t-test. The proposed evidence-based osteoporosis exercise education program will be considered as effective if (1) there is a reduction of bone loss and an increase in physical activity level in postmenopausal women, and (2) there is an increase of confidence and self-perceived skill on the prevention of management of osteoporosis in nurses. Book excerpt: Longer longevity in women is imminently associated with deterioration of functional activity driven by menopausal transition. However, at present there is a dearth of data on means of modulation, other than pharmaceutical, of menopause associated detrimental phenomena. This study is an amalgam of my previous study and the new analysis examining menopause associated changes in a variety of health influencing parameters such as body mass index, waist circumference, total cholesterol, triglycerides, high-density cholesterol, low-density cholesterol, luteinizing and follicle stimulation hormones, blood pressures, serum uric acid levels, and serum creatinine. Through a combination of the novel data and an extensive literature review I postulate that physical exercises-including endurance, aerobic, and strength exercises - may be used as a happy medium for preserving a high level of Health Related Quality of Life in elderly and menopausal women. I demonstrated that physical exercises used as prevention means for detrimental changes should not be focused on an increase in a fitness level but on a decrease of a total body fat.

Book excerpt: To measure quality of life (QOL) in postmenopausal women with osteoporosis, using Qualeffo41 and to examine factors influencing quality of life in postmenopausal women with osteoporosis. A cross-sectional descriptive study investigated quality of life in 45-year-old and older postmenopausal women with low bone mineral density (BMD) (≤ -2.5 standard deviations (S.D.) below normal peak bone mass) or were diagnosed as an osteoporosis or taking osteoporosis drug. A total of 134 subjects were recruited from the Police General Hospital during December 2009 to March 2010. The interview-administered questionnaire consists of 4 parts which are demographic and clinical characteristics, quality of life, factors in health concerning and factors in treatment. The mean, standard deviation, range and multiple regression analysis were used for statistical analysis. Quality of life assessment was performed using the Qualeffo41 (Thai version) which express in values ranging from 0 to 100, where 0 represents the best and 100 represents the worst quality of life. Results showed that the mean (±S.D.) total QOL score of postmenopausal women with osteoporosis was 34.86 (±15.67). In each domain, the result showed that the score of pain domain was 30.34 (±25.27), physical function domain was 36.82 (±21.52), social activities domain 44.53 (±22.35), general health perception domain was 44.47 (±18.53) and mental domain scored was 24.60 (±15.59). The patient's QOL was affected by four predictor variables which are 1) occupation, 2) duration of menopause, 3) exercise, and 4) dietary calcium and protein intake. Duration of menopause related to worse QOL in the patients. While exercise and dietary calcium and protein intake related to better QOL in the patients. Thus, getting more exercise or consuming more calcium and protein diet is recommended in order to have better quality of life.

A lack of activity increases the risk of heart disease, colon and breast cancer, diabetes mellitus, hypertension, osteoporosis, anxiety and depression and others diseases. Emerging literature has suggested that in terms of mortality, the global population health burden of physical inactivity approaches that of cigarette smoking. The prevalence and substantial disease risk associated with physical inactivity has been described as a pandemic. The prevalence, health impact, and evidence of changeability all have resulted in calls for action to increase physical activity across the lifespan. In response to the need to find ways to make physical activity a health priority for youth, the Institute of Medicine's Committee on Physical Activity and Physical Education in the School Environment was formed. Its purpose was to review the current status of physical activity and physical education in the school environment, including before, during, and after school, and examine the influences of physical activity and physical education on the short and long term physical, cognitive and brain, and psychosocial health and development of children and adolescents. Educating the Student Body makes recommendations about approaches for strengthening and improving programs and policies for physical activity and physical education in the school environment. This report lays out a set of guiding principles to guide its work on these tasks. These included: recognizing the benefits of instilling life-long physical activity habits in children; the value of using systems thinking in improving physical activity and physical education in the school environment; the recognition of current disparities in opportunities and the need to achieve equity in physical activity and physical education; the importance of considering all types of school environments; the need to take into consideration the diversity of students as recommendations are developed. Book excerpt: There are a number of therapies and treatments available for the prevention of fragility fractures in people thought to be at risk, or to prevent further fractures in those who have already had one or more fragility fractures. However, identifying who will benefit from preventative treatment is imprecise. A number of risk assessment tools are available to predict fracture incidence over a period of time, and these may be used to aid decision making. These tools are limited in that they may not include all risk factors, or may lack details of some risk factors. Tools are dependent on the accuracy of the epidemiological data used to derive them and tools validated in other populations may not apply to the UK. Two tools, FRAX and QFracture, are available for use in the UK. It is not clear whether these tools are equally accurate and whether choice of tool should depend on circumstances. This short clinical guideline aims to provide guidance on the selection and use of risk assessment tools in the care of people who may be at risk of fragility fractures in all settings in which NHS care is received.
